Hypertension and its Dietary Mangement by our Nutrition expert - Dr.Varsha Joshi

What is Hypertension?

Hypertension is high blood pressure, medically defined as - Blood pressure which remains consistently high and not just once / twice is termed hypertension.

 Blood pressure is measured in two parameters. One is systolic (upper) and second is diastolic (lower), 120/80 mm of mercury, is the normal reading.

 Causes:

  • In most cases the cause for high blood pressure is not known.
  • Stress.
  • Obesity
  • Family heredity.
  • Hormonal imbalance.
  • Kidney Diseases

What we eat directly affects our blood pressure. Hence it is very important to follow a strict dietary and an exercise regime. Here are some dietary tips to manage hypertension:

  • Cut down on salt (common table salt) intake; do not exceed more then 2 to 2.5 gm a day.
  • In cases of severe hypertension avoid other sources of salts like- baking powder and baking soda (sodium bicarbonate), monosodium glutamate and sodium benzoate. These are commonly known as Worcestershire sauce, soy sauce, onion salt, garlic salt and buillon salt. Also avoid salt-preserved foods like pickles, sauces, ketchups, canned and processed foods.
  • Consume food rich in magnesium  and calcium like – milk and milk products, wheat, brown rice, millets, green leafy vegetables, Sea plants, peas, lotus stem, pulses, legumes, crab, oyster, shellfish, nuts like - walnut, almond(are most concentrated source of magnesium but should be consumed in a very limited quantity as they are high in fats).
  • Eat one banana a day will help control hypertension as it is high in fiber, potassium, calcium, phosphorus and magnesium.
  • Reduce intake of diet rich in saturated fats e.g. oily, fired food, desserts, red meat, eggs yolk, cheese, butter etc.
  • Avoid foods rich in cholesterol: groundnut oil, ghee, butter, whole milk, eggs, lard (fats derived from pig meat), tallow (fats derived from goat or sheep mutton), oily fish, meats and red meat - all non-vegetarian food contain cholesterol in varied amount.
  • Increase intake of foods that contain PUFA's (poly unsaturated fatty acids): flax / linseed oil is the richest source of PUFA's. Other oils that contain PUFA's are safflower oil, soybean oil, cottonseed oil, maize oil, sesame oil, rich bran oil, ground nut oil, mustard oil and olive oil. As all oils are high in fats, its consumption should be limited. The intake of oils should not exceed 20gms a day.
  • Consume whole grain cereals, whole pulses, fresh fruits and vegetables – as they are high in fiber.
  • Avoid coffee and caffeinated drinks.
  • Walk every day briskly for at least 30 minutes.
  • Meditation & yoga will help to reduce and tackle stress.

Facts about Tuberculosis

·         Tuberculosis (TB) is a contagious disease caused by the bacteria known as Mycobacterium tuberculosis which spreads through air. It is transmitted from person to person via droplets from the throat and lungs of people with the active respiratory disease. If TB is untreated each person suffering from active tuberculosis can infect approximately 10 -15 persons in a year.

·         TB is a disease which is seen in the lower socio economic sections of the society but this does not rule out the incidence of TB in higher socio economic groups.TB mostly affects young adults in their productive years and vast TB deaths are in the developing world, more then half occurring in Asian countries.

·         TB is a worldwide pandemic. Among the 15 countries with the highest estimated TB incidence rates, 13 are in Africa, while a third of all new cases are in India and China.(WHO)

·         Multidrug-resistant TB (MDR-TB) is a form of TB that does not respond to the standard treatments using first-line drugs. Extensively drug-resistant TB (XDR-TB) occurs when resistance to second-line drugs develops. It is extremely difficult to treat.

·         TB is a major disease that affects person living with HIV as they have a weakened immune system. Due to the weakened immunity and virulence of TB it is a leading killer among people suffering from HIV infection.
·         TB can be of various organs like pulmonary tuberculosis affecting the lungs, Tubercular lymphadenopathy affecting the lymph nodes, abdominal tuberculosis, Tuberculoma affecting the brain, Tubercular meningitis to name a few. It is very important to treat tuberculosis and the proper treatment has to be taken.

      Extensively drug-resistant tuberculosis

      XDR-TB is the abbreviation for extensively drug-resistant tuberculosis (TB). One in three people in the world is infected with dormant TB germs. Only when the bacteria become active do people become ill with TB.
      Bacteria become active as a result of anything that can reduce the person’s immunity, poor nutritional status, infections as HIV, advancing age, or other medical conditions.TB can usually be treated with a course of first-line, anti-TB drugs. If these drugs are misused or mismanaged, multidrug-resistant TB (MDR-TB) can develop.
      
      MDR-TB takes longer time to treat with second-line drugs, which are more expensive and have more side-effects. XDR-TB can develop when these second-line drugs are also misused or mismanaged and therefore also become ineffective. Because XDR-TB is resistant to first- and second-line drugs, treatment options are seriously limited in XDR - TB. It is therefore vital that TB control is managed properly.

Low Back pain - Homeopathic treatment


Low back pain can be differentiated into four types :  

a)      Local pain
b)      Referred pain
c)      Radicular pain
d)      Pain arising from secondary muscle spam (A protective mechanism)

There are several symptoms in the diseases of the spine. The person may experience stiffness, limitation in the movement and deformity but pain gets the foremost importance because of discomfort and the disability that is experienced by the suffering person.

Importance must be given to the character, location and the conditions that modify the pain. This will help in correctly identifying the cause of the pain.

Local pain

It is caused due to pathologic process, which presses upon or irritates the sensory nerve endings (Sensory nerve endings the nerve endings which carry the pain sensation). If the involved structure doesn’t have sensory nerve endings then there will be no pain.

For e.g. if the central medullary part of the vertebral body is destroyed by a tumor there will be no evidence of pain due to absence of sensory nerve fibres. In the same case if the affected parts are the muscles, the ligaments or the annulus fibrosis it will cause severe pain due to involvement of the sensory nerve endings.

Often there will be swelling of the tissues involved in case of pain but it cannot be noticed if the deep structures of the back are involved. The character of the pain is described as a steady intermittent but it can also be a sharp and dull pain. The pain varies considerably with the position and the activity of the spine.

One characteristic of the local pain is that it will be felt in the area that is affected. The patient will experience tenderness if the affected part is examined by putting firm pressure. This helps in identifying the site of abnormality.

Referred pain
As the name suggests, “referred” it is the pain that is referred to other part of the body from the site of abnormality. This happens due to the fact that the nerve that is affected also supplies to the other structures of the body. So if the nerve is compressed or impinged the pain will also be felt in the distant parts that are supplied by the same nerve.

Referred pain is of two types one type of pain is that which is projected from the spine into the regions, which are supplied by the nerves that pass from the lumbar or sacral region of the spine (Lumbar and upper sacral dermatomes).

The other type of referred pain is the one, which is referred from the abdominal structures to the spine.

If the upper part of the lumbar spine is affected the pain will be experienced in the anterior aspect of the thighs and the legs. If the lower lumbar and sacral segments are involved then the pain will be referred to the gluteal region (Buttocks) and the posterior aspect of the thighs and the calves.

The character of the referred pain is of deep aching character and is diffuse. At times the pain is felt in the superficial structures of the back. Certain movements, which give relief in the local back pain, will also relieve the referred pain.

Relief of Radicular or root pain is more precise with certain movements of the spine, which release the pressure on the affected nerves. Referred pain can sometimes be confused with the pain in abdomen but the abdominal pain will be deep and the radiation will be from the abdomen to the back.

Also if the pain is due to any pathology in the abdominal organs there wont be much change in the pain on movement as seen in referred pain. Whereas relief will be felt when there is activity of the affected part.


Radicular pain or Root pain

As the name suggests it is caused by irritation, stretching or compression of the spinal root (other words when the nerve roots are affected). The Radicular pain has some of the characteristics of the referred pain but it differs from it by the intensity of the pain and the localized pain that is experienced. Also there will be radiation of pain to the fingers.

The pain is often of a dull aching type but if the nerve roots are compressed or irritated due to certain movements of the spine the pain can be very severe. Most of the time the radiation of pain is from the central position of the spine to the lower extremity. 

Coughing, sneezing and certain movements will evoke severe pain as these things put
strain on the spine. Any motion that stretches the affected nerves like the forward bending with the knees extended or straight leg raising in diseases of the lumbar spine will excite Radicular pain.

The fourth, fifth lumbar and the first sacral roots which form the sciatica nerve when affected causes pain which mainly extends down the posterior aspect of the thigh, the posterior and anterolateral aspect of the leg and into the foot as these parts are supplied by the sciatic nerve this condition is known as Sciatica.

The other complaints that accompany the radicular pain are tingling, Parestheasia, Numbness, Sensory impairment of the skin, Soreness and tenderness of the skin along the course of the nerve is present. There can also be loss of reflexes, weakness, wasting of muscle, fascicular twitching and often swelling of the leg (Stasis edema) may occur if the motor fibers of the anterior root of the spinal cord are involved.

The pain resulting from muscular spasm is most of the times mentioned in relation to the local pain. Muscle spasm maybe associated with various diseases of the spine. Due to the spasm there will be significant change in the posture of the person. The person experiences dull, cramping pain due to chronic tension in the muscles.

The person suffering from chronic disease of the lower back may experience pain, which cannot be determined in some cases. There can be pulling, drawing, cramping sensation without involuntary muscle spasm along with this there can be tearing, burning and throbbing pains. The person experiences coldness or burning which is often difficult to interpret but the possibility of Nerve or root disease should be kept in mind.

In a case of back pain it is very important to obtain a correct history along with the character, Location and radiation of the pain. The factors, which aggravate and ameliorate the pain and its consistency, should also be taken into consideration.

The physician should also take into consideration the past history, which might give vital clues like accidents or injuries that have been incurred. Also a watch should be kept on people who are hysterical and who are pretending to have severe pain for getting compensation.

The role of homeopathy lies in the fact that it can bring back the lost balance in the human body when it is in a diseased state.

International day of persons with disabilities – 3rd December 2011


Did you know that according to the new report there are more 1 billion people across the globe face substantial  barriers in their daily lives.The day aims to promote an understanding of disability issues and mobilize support for the dignity, rights and well-being of persons with disabilities. It also seeks to increase awareness of gains to be derived from the inclusion of persons with disabilities in every aspect of life.
Lets see which countries are user friendly for people who suffer from disabilities .One of the main concern that I always felt was how people with disabilities travel ? With the growing modernization and the population explosion in  countries like India ,China etc .Here modern infrastructure is being built but in India I have seen minimal thought given to the disabled people.

When I travel by the local train to reach the platform from where the train leaves its just impossible for a wheelchair bound person to access this service.Now since few years there is a small compartment which is specially for the disabled and also for the cancer patients.This is a good initiative by the government .
The public transport buses have 4 -5 seats reserved for the disabled but here also there is no facility for a wheel chair bound person.Last week when I was traveling in the BEST bus there was a heart breaking incident a mother was sitting with her son who was suffering from cerebral palsy and was also blind and a person came who was also little disabled he started fighting with that lady that she should vacate the seat .The lady explained that it will be impossible to leave the child sit alone but the other person didn’t understand the complex disease the child was suffering from .

Looking at this whole thing a fellow passenger offered his seat.The point is there has to be awareness campaigns where people understand how to deal with disabilities and also a board has to be put where care takers can seat with the disabled people.

Some of the best practices that I have encountered in India where disabled people are concerned is the outlook of the general people.Indian people are very helpful when they see a disabled person who needs any help like while crossing streets, some organizations are doing tremendous work,also the students from schools and colleges are very friendly and helpful.

Filling the Holes: How Homeopathy Deserves a Place in Cancer Treatment by Guest blogger Allison Brooks


Homeopathy is being in practiced for more then 200 years  .  Before there were “white-coat doctors” and hospitals, there was nature. Nature provided and still provides everything needed to sustain life. Nature gives life, food, water, and health. But as time goes on, humans separate themselves with nature more and more, and the trust in using natural remedies is nonexistent. But, if people looked at the facts, their trust in conventional therapies and medications would diminish.

This is where the example of “filling the holes” comes in; what better way to explain a medical therapy from nature, than with an analogy from nature. In 2010, there were 2.7 million new HIV infections and 1.8 million people died from AIDS. Moreover, in 2011 people living with HIV were still subject to restrictions on their travel and/or stay in 47 countries, territories and areas.

Brief History of WORLD AIDS DAY

World AIDS Day was first conceived in August 1987 by James W. Bunn and Thomas Netter, two public information officers for the Global Programme on AIDS at the World Health Organization in Geneva, Switzerland. Bunn and Netter took their idea to Dr. Jonathan Mann, Director of the Global Programme on AIDS (now known as UNAIDS). Dr. Mann liked the concept, approved it, and agreed with the recommendation that the first observance of World AIDS Day should be 1 December, 1988.

Bunn, a broadcast journalist on a leave-of-absence from his reporting duties at K PIX-TV in San Francisco, recommended the date of 1 December believing it would maximize coverage by western news media. Since 1988 was an election year in the U.S., Bunn suggested that media outlets would be weary of their post-election coverage and eager to find a fresh story to cover. Bunn and Netter determined that 1 December was long enough after the election and soon enough before the Christmas holidays that it was, in effect, a dead spot in the news calendar and thus perfect timing for World AIDS Day.

In 2004, the World AIDS Campaign became an independent organization.

It was 30 years ago this week that federal health officials first reported on a rare and deadly illness they called AIDS. Since then millions have died of it, including these celebrities. 

A leading man in the romantic comedies of the 1950s and 1960s, actor Rock Hudson was among the first celebrities to die of AIDS-related diseases. His Oct. 2, 1986, death, just before his 40th birthday, led his friend and "Giant" co-star Elizabeth Taylor to spend the rest of her life raising funds for AIDS research.

1. What is autism? 

Autism is considered a spectrum disorder because symptoms and severity vary from individual to individual. Autistic children express differently from each other. Autism is characterized by failure of the child to develop communicative language or form proper social communication. But they show motor and other skills far beyond that of a mentally retarded child.

Autism is characterized by varying degrees of impairment to develop communicative language or form proper social communication. Some of them are obsessively pre occupied with inanimate objects such as lights, running water or spinning objects, and also by repetitive behaviors. Symptoms range from mild to severe. Some of them gradually develop extreme talents such as in mathematics and these are retained mostly throughout the life. Studies of people with autism have found abnormalities in several regions of the brain which suggest that autism results from a disruption of early fetal brain development

There is a milder form of the disorder known as Asperger's Syndrome. Other developmental disorders that fall under the Autism Spectrum Disorders are Rett’s Syndrome, PDD-NOS (Pervasive Developmental Disorder, Not Otherwise Specified), and Childhood Disintegrative Disorder. 2. What is meant by “On the spectrum”?
Autism is considered a spectrum disorder because symptoms and severity vary from individual to individual. Though the Autistic children show similar traits they are individual and express differently from each other. one child may have not developed speech and have difficulty in learning how to read and write, where as another child can easily read and write and can be attending normal school. Some children may display hypersensitivity to touch noise and other may have no sensory issues at all.

3. How common is autism?
According to the survey by Centers for Disease Control, autism affects as many as 1 in every 100 children and it is rising by 10-15% every year. It is the most prevalent developmental disorder in today’s time. Boys are more susceptible than girls to develop Autism but it is seen that girls manifest a more severe form of the disorder that the boys.

4. How do I know my child is Autistic?
No two children with autism are alike, each one managing a different set of symptoms. But there are some signs and symptoms that are common and may be recognized as early as the toddler years, or even sooner. They usually do not have or do not sustain eye contact and have trouble communicating with other people or they may communicate in ways that others cannot easily understand. They may hardly speak and even if they speak it’s just babbling and not coherent. They're also prone to repetitive behaviors like flapping their hands constantly or uttering the same phrase over and over again. They may also be over sensitive to sights, sounds and touch.

Criteria for Autism
A total Of 6 or more items of the following with at least 2 from [1], and one each from [2] and [3] are satisfied by the patient:

[1] Qualitative impairment in social interaction:

Marked impairment in facial expression
Failure to develop peer relationships
A lack of spontaneity in sharing interests
Lack of social or emotional reciprocity

[2] Qualitative impairment in communication:
Marked inability to initiate or sustain conversation with others.

[3] Restricted, repetitive and stereotyped pattern of behavior
Encompassing preoccupation with one stereotyped and restricted pattern.


5. How did my child develop autism?
No one is yet able to point to a probable cause. Though a disorder as common as autism would have a known cause, in many ways it's still quite mysteriously not known. Recent studies have suggested a strong genetic basis for autism. There are about 20 sets of genes that may play a part in the development of autism. Genes are not the only one responsible for all the cases; there are other triggers which are yet not discovered.

6. Does vaccination cause autism ?
Many studies have been conducted to determine if a link exists between vaccination and increased prevalence of autism, with particular attention to the measles-mumps-rubella (MMR) vaccine and those containing thimerosal. These studies have not found a link between vaccines and autism It remains possible that, in cases, immunization might trigger the onset of autism symptoms in a child with an underlying medical or genetic condition.

As this is a sensitive topic it is best to discuss it with your healthcare provider who will guide you and ensure the optimal well-being of the child. Establishing open communication and trust with a physician who understands each child and his or her family is the best strategy for keeping a child healthy.
