Introduction:

We are not aware of how much time we spend with the computer and keyboard. Carpal tunnel syndrome occurs due to compression of the median nerve as it passes through the carpal tunnel in the flexor retinaculum at the wrist.

Bounded by bones and ligaments, the carpal tunnel is a narrow passageway located on the palm side of your wrist. This tunnel protects a main nerve to your hand and nine tendons that bend your fingers. Pressure placed on the nerve produces the numbness, pain, and weakness in the hand that characterize carpal tunnel syndrome.

Cause:

• Wrist fracture
• Working on a keyboard, typing, carpentry and other tasks that require repetitive motions of wrists and hands are the common causes.
• Repetitive flexing and extending of the tendons in the hands and wrists when done forcefully for prolonged periods can increase the pressure within the carpal tunnel.
• Arthritis of the wrist particularly rheumatoid arthritis.
• Soft tissue thickening in myxoedema and acromegaly.
• Edema associated with pregnancy.
• Idiopathic.
• Obesity.
• Sometimes if your carpal tunnel is more narrow than average.

Signs and Symptoms:

• Pain, numbness, and paraesthesia in hand. It usually starts with a vague aching in the wrist that can extend to the hand or forearm.
• Pain may radiate through the forearm and occasionally involve the whole arm.
• Tingling and numbness in your fingers or hand, especially the thumb, index, middle or ring fingers. This usually occurs while holding a phone or while driving or simply after awakening.
• Typically pain is more at night or early in the morning.
• Weakness of the muscles of hand with or without wasting. Tendency to drop objects.
• Positive Tinel sign - Gentle tapping over carpal tunnel causes paraesthesia in part of the cutaneous distribution of the nerve.

Complications:

If left untreated damage can be caused to the muscles and nerves.

Diagnosis:

• Diagnosis is made on the basis of signs and symptoms by a physician. The median nerve does not provide sensation to the little finger, so that finger is not involved.
• Your doctor will also want to test the sensation in your fingers and the strength of the muscles in your hand. Pressure on the median nerve at the wrist, produced by either bending the wrist, tapping on the nerve or simply pressing on the nerve, can bring on the symptoms in many people. The following diagnostic tests are recommended.
• Electromyography measures the tiny electrical discharges produced in muscles. A thin-needle electrode is inserted into the muscles your doctor wants to study. An instrument records the electrical activity in your muscle at rest and as you contract the muscle. This test determines if muscle damage has occurred.
• A nerve conduction study is carried out. Two electrodes are taped to your skin. A small shock is passed through the median nerve to see if electrical impulses are slowed in the carpal tunnel.

Prevention:

• Move your fingers; perform some exercises to relax your fingers, especially after typing or doing repetitive work.
• Take small breaks after working continuously. Gently bend and stretch your hands and wrists.
• Correct your posture as improper posture may compress the nerves in your neck which can affect wrists' fingers and hands. Avoid bending your wrist all the way up or down. The keyboard should be at elbow height or lower.
• Wearing fingerless gloves can keep the wrist and hand warm and prevent stiffness if you are working in a cold environment.

Treatment:

The conventional form of treatment includes a non-surgical approach like steroids, wrist splints, and pain killers. If the symptoms persist or become severe then a surgical approach is taken.

Acid and Alkaline food

It is essential to balance the acid and alkali levels in our bodily systems.The body has internal mechanism to balance the acid alkaline ratio but it is important to have a balanced acid alkaline food. The pH factor indicates the acidity and alkalinity of the body and plays a vital role in body chemistry. Normal pH value is 7 and under this indicates acidity, while values above 7 are an indicator of alkalinity.

The pH value of saliva and urine is 6 and 6.8 respectively which shows that they are slightly acidic in nature. These are the average values; any value above this would indicate alkaline nature, while anything lower would indicate a more acidic nature. A 3 years 2 months old child suffering from severe sensory issues due to autism was bought to our consultation room for homeopathic treatment. The child was putting everything in mouth and licks whatever e gets in his hand. The child also handles his private parts and if there is any noise he will cover his ears and he becomes uneasy.

The child also eats only specific food and dislikes any change in texture of the food; he also doesn't chew the food and mostly swallows the food.

The child is very restless and doesn't sit in one place he constantly runs from one place to another and the focus is also very less. When the child is taken for therapy the major issue is that the child doesn't concentrate or sit in one place. The child keeps smiling for no reason there is lot of anger present when he doesn't get what he wants. When angry the child will bite and pull hair and doesn't get pacified easily.

The child doesn't play with children of his age and there is no speech till now. The child loves to play in water and can spend hours together playing in water. Developmental Milestones

Age of walking: 2 years Eruption of first teeth: 6 months the child used to have dentitional diarrhea which was treated with Homeopathy. Speech: Not yet developed.

The mother had no tensions during pregnancy except bleeding during the 3rd month of pregnancy but it was treated and she was alright.The pregnancy was planned and there is no history of any major illness in the parents.

Personal history:

• Diet: Vegetarian
• Appetite: Good
• Cravings: Ice, Ice cream, tooth paste
• Aversion ::textures, if the milk is changed then he will have diarrhea
• Thirst: For large quantities of water.
• Sleep and Dreams: sleeps in knee chest position, grinding of teeth in sleep earlier. Sleep is very less and is very restless and takes lot of time to fall asleep.
• Urine: Normal
• Stools: Normal. If he doesn't pass stools for 1 day he will become very restless.
• Thermals:the child cannot bear heat and feel very hot .Even in cold weather he doesn't feel that cold.

Family history: No major illness

Remedy Selection and Follow ups

The remedy was selected after analysis and evaluation of the case history. The homeopathic remedy was prescribed in the 30c potency. The name of the remedy is not disclosed to prevent self medication. The patient was give 1 dose at bedtime for 14 days and the parents were asked to report. As the patient was from out station the next follow up was scheduled via video conferencing.

The parent reported that the putting of things in mouth is less. The child feels very hot and wants to be in cool place. The sleep is improved and he doesn't take much time to sleep. Hyperactivity is the same. When he goes out he doesn't like to walk. The licking of objects is also decreased. The same medicine was continued and the follow up was scheduled after a month. The parents were asked to report regularly on phone.

Biting in anger, doesn't cry now, sleep is better, but the anger has increased. Sensory issues are getting better. Babbling has increased. Eating is better now, chews food. Note should be made that in some cases we see rapid improvement but the results vary from case to case depending on the person's susceptibility and the intensity of the disorder. The same medicine was continued the potency was raised to 200c.

Crying since today morning as his uncle scolded him. This was a good improvement and the parents were happy as he could comprehend that someone is angry. Also the babbling has increased and he attempts to talk and puts pressure. Pulling of hair, biting, and anger have reduced markedly. The sensitiveness to sound where he used to cover his eyes is not marked now. The touching of private parts has decreased. His concentration during therapy is better and can focus better than before.

Case of  autism (Case 1)

An interesting case of a young boy staying in United States diagnosed with Autism. This boy was very talented and he has exemplary skills in playing the piano but was very hyperactive and the attention span was very less. There was lack of comprehension, certain sensory issues, putting the hands to the ears, arranging things in one order, lack of eye contact were few of the major concerns.

A severe case of Autism( Case 2)

This is a case of 11 years old girl who is a severe case of autism there is absolutely no social interaction the child is lost in her own world. The child is very close to nature and she is very comfortable when she is out in the open .She will move around the park and will be very peaceful. She also loves to be on swings and just loves the rocking movement that is to and fro.

The main cause of concern in this child was the severe head banging she would do when she didn’t get what she wanted or it would be without any relevant cause. The head banging was so severe that it used to cause swelling on the head.

There was also a separation anxiety and she is very attached to her parents and was also attached to a maid who unfortunately quit the job and this had also led to lot of adjustment problems with the new maids. She used to spit and bite the new maids as they didn’t understand what she actually wanted.

The eating habits are also very poor she takes selected food besides that she won’t take anything else. Adjusting to a new place or a new person is also very problematic. She loves to listen to music and will also hum few of the songs. Speech is limited to very few words, and she follows few commands but this is also very minimal.

The remedy that was prescribed and which helped was Kali.carb it was given in the 200c potency. After giving this the head banging has stopped. The child is still under our treatment.


The Happy Child( Case 3 )

This child was very cheerful when he came to our consultation he is a 3 years old boy. The parents consulted us after reading the article on how homeopathy can help in Autism.

In this case the parents were very anxious about what holds for the child in the future. They wanted to know if their son will ever talk again as it was traumatic for them to accept that a child who was talking suddenly stops speaking anything.

The parents were even more worried after they visited a therapist who just told them about autism without explaining that there is always a future with therapy and treatment.

The child was very hyperactive and was running from one corner to the other constantly, there was minimal eye contact and he didn’t obey commands at all. He also had the habit of spitting and playing with the spit, there was no grip for holding a pen or pencil. The child had tremendous fear of darkness; he also played with his genitals which used to cause lot of embarrassment to the parents.

The remedy that was prescribed was Strammonium LM 3 this was his constitutional remedy we later followed it up with Bacillinum after few months when there was no progress happening in the case
