· Tuberculosis (TB) is a contagious disease causedby the bacteria known as Mycobacterium tuberculosis which spreads through air. Itis transmitted from person to person via droplets from the throat and lungs ofpeople with the active respiratory disease. If TB is untreated each personsuffering from active tuberculosis can infect approximately 10 -15 persons in ayear.

· TB is a disease which is seen in the lower socioeconomic sections of the society but this does not rule out the incidence of TBin higher socio economic groups.TB mostly affects young adults in theirproductive years and vast TB deaths are in the developing world, more then halfoccurring in Asian countries.

· TB is a worldwide pandemic. Among the 15countries with the highest estimated TB incidence rates, 13 are in Africa,while a third of all new cases are in India and China.(WHO)

· Multidrug-resistant TB (MDR-TB) is a form of TBthat does not respond to the standard treatments using first-line drugs.Extensively drug-resistant TB (XDR-TB) occurs when resistance to second-linedrugs develops. It is extremely difficult to treat.

· TB is a major disease that affects person livingwith HIV as they have a weakened immune system. Due to the weakened immunity andvirulence of TB it is a leading killer among people suffering from HIVinfection.

· TB can be of various organs like pulmonary tuberculosisaffecting the lungs, Tubercular lymphadenopathy affecting the lymph nodes, abdominaltuberculosis, Tuberculoma affecting the brain, Tubercular meningitis to name a few.It is very important to treat tuberculosis and the proper treatment has to betaken.

Extensively drug-resistant tuberculosis

XDR-TB is theabbreviation for extensively drug-resistant tuberculosis (TB). One in threepeople in the world is infected with dormant TB germs. Only when the bacteriabecome active do people become ill with TB.

Bacteria becomeactive as a result of anything that can reduce the person’s immunity, poornutritional status, infections as HIV, advancing age, or other medicalconditions.TB can usually be treated with a course of first-line, anti-TBdrugs. If these drugs are misused or mismanaged, multidrug-resistant TB(MDR-TB) can develop.

MDR-TB takeslonger time to treat with second-line drugs, which are more expensive and havemore side-effects. XDR-TB can develop when these second-line drugs are alsomisused or mismanaged and therefore also become ineffective. Because XDR-TB isresistant to first- and second-line drugs, treatment options are seriouslylimited in XDR - TB. It is therefore vital that TB control is managed properly.

Low back pain can bedifferentiated into four types :

a) Local pain

b) Referred pain

c) Radicular pain

d) Pain arising from secondary muscle spam (A protectivemechanism)

Importance must be given to thecharacter, location and the conditions that modify the pain. This will help incorrectly identifying the cause of the pain.

Local pain

It is caused due to pathologicprocess, which presses upon or irritates the sensory nerve endings (Sensorynerve endings the nerve endings which carry the pain sensation). If theinvolved structure doesn’t have sensory nerve endings then there will be nopain.

For e.g. if the central medullarypart of the vertebral body is destroyed by a tumor there will be no evidence ofpain due to absence of sensory nerve fibres. In the same case if the affectedparts are the muscles, the ligaments or the annulus fibrosis it will causesevere pain due to involvement of the sensory nerve endings.

Oftenthere will be swelling of the tissues involved in case of pain but it cannot benoticed if the deep structures of the back are involved. The character of thepain is described as a steady intermittent but it can also be a sharp and dullpain. The pain varies considerably with the position and the activity of thespine.

One characteristic of the localpain is that it will be felt in the area that is affected. The patient willexperience tenderness if the affected part is examined by putting firmpressure. This helps in identifying the site of abnormality.

Referred pain

As the name suggests, “referred”it is the pain that is referred to other part of the body from the site ofabnormality. This happens due to the fact that the nerve that is affected alsosupplies to the other structures of the body. So if the nerve is compressed orimpinged the pain will also be felt in the distant parts that are supplied bythe same nerve.

Referred pain is of two types onetype of pain is that which is projected from the spine into the regions, whichare supplied by the nerves that pass from the lumbar or sacral region of thespine (Lumbar and upper sacral dermatomes).

The other type of referred painis the one, which is referred from the abdominal structures to the spine.

If the upper part of the lumbarspine is affected the pain will be experienced in the anterior aspect of thethighs and the legs. If the lower lumbar and sacral segments are involved thenthe pain will be referred to the gluteal region (Buttocks) and the posterioraspect of the thighs and the calves.

The character of the referred painis of deep aching character and is diffuse. At times the pain is felt in thesuperficial structures of the back. Certain movements, which give relief in thelocal back pain, will also relieve the referred pain.

Relief of Radicular or root painis more precise with certain movements of the spine, which release the pressureon the affected nerves. Referred pain can sometimes be confused with the painin abdomen but the abdominal pain will be deep and the radiation will be fromthe abdomen to the back.

Also if the pain is due to anypathology in the abdominal organs there wont be much change in the pain onmovement as seen in referred pain. Whereas relief will be felt when there isactivity of the affected part.

Radicular pain or Root pain

As the name suggests it is causedby irritation, stretching or compression of the spinal root (other words whenthe nerve roots are affected). The Radicular pain has some of thecharacteristics of the referred pain but it differs from it by the intensity ofthe pain and the localized pain that is experienced. Also there will beradiation of pain to the fingers.

The pain is often of a dullaching type but if the nerve roots are compressed or irritated due to certainmovements of the spine the pain can be very severe. Most of the time theradiation of pain is from the central position of the spine to the lowerextremity.

Coughing, sneezing and certainmovements will evoke severe pain as these things put

strain on the spine. Any motionthat stretches the affected nerves like the forward bending with the kneesextended or straight leg raising in diseases of the lumbar spine will exciteRadicular pain.

The fourth, fifth lumbar and thefirst sacral roots which form the sciatica nerve when affected causes painwhich mainly extends down the posterior aspect of the thigh, the posterior andanterolateral aspect of the leg and into the foot as these parts are suppliedby the sciatic nerve this condition is known as Sciatica.

The other complaints thataccompany the radicular pain are tingling, Parestheasia, Numbness, Sensoryimpairment of the skin, Soreness and tenderness of the skin along the course ofthe nerve is present. There can also be loss of reflexes, weakness, wasting ofmuscle, fascicular twitching and often swelling of the leg (Stasis edema) mayoccur if the motor fibers of the anterior root of the spinal cord are involved.

The pain resulting from muscularspasm is most of the times mentioned in relation to the local pain. Musclespasm maybe associated with various diseases of the spine. Due to the spasmthere will be significant change in the posture of the person. The personexperiences dull, cramping pain due to chronic tension in the muscles.

The person suffering from chronicdisease of the lower back may experience pain, which cannot be determined insome cases. There can be pulling, drawing, cramping sensation withoutinvoluntary muscle spasm along with this there can be tearing, burning andthrobbing pains. The person experiences coldness or burning which is oftendifficult to interpret but the possibility of Nerve or root disease should bekept in mind.

In a case of back pain it is veryimportant to obtain a correct history along with the character, Location andradiation of the pain. The factors, which aggravate and ameliorate the pain andits consistency, should also be taken into consideration.

The physician should also takeinto consideration the past history, which might give vital clues likeaccidents or injuries that have been incurred. Also a watch should be kept onpeople who are hysterical and who are pretending to have severe pain forgetting compensation.

Did you know that according to the new report there are more1 billion people across the globe face substantial barriers in their daily lives.The day aims topromote an understanding of disability issues and mobilize support for thedignity, rights and well-being of persons with disabilities. It also seeks toincrease awareness of gains to be derived from the inclusion of persons withdisabilities in every aspect of life.

Lets see which countries are user friendly for people whosuffer from disabilities .One of the main concern that I always felt was howpeople with disabilities travel ? With the growing modernization and the populationexplosion in countries like India ,Chinaetc .Here modern infrastructure is being built but in India I have seen minimalthought given to the disabled people.

When I travel by the local train to reach the platform fromwhere the train leaves its just impossible for a wheelchair bound person toaccess this service.Now since few years there is a small compartment which isspecially for the disabled and also for the cancer patients.This is a goodinitiative by the government .

The public transport buses have 4 -5 seats reserved for the disabled but here also there is no facility for a wheel chair bound person.Lastweek when I was traveling in the BEST bus there was a heart breaking incidenta mother was sitting with her son who was suffering from cerebral palsy andwas also blind and a person came who was also little disabled he startedfighting with that lady that she should vacate the seat .The lady explainedthat it will be impossible to leave the child sit alone but the other person didn’tunderstand the complex disease the child was suffering from .

Looking at thiswhole thing a fellow passenger offered his seat.The point is there has to beawareness campaigns where people understand how to deal with disabilities andalso a board has to be put where care takers can seat with the disabled people.

Some of the best practices that I have encountered in Indiawhere disabled people are concerned is the outlook of the general people.Indianpeople are very helpful when they see a disabled person who needs any help likewhile crossing streets, some organizations are doing tremendous work,also thestudents from schools and colleges are very friendly and helpful.

In 2010, there were 2.7 million new HIV infections and 1.8 millionpeople died from AIDS. Moreover, in 2011 people living with HIV were stillsubject to restrictions on their travel and/or stay in 47 countries,territories and areas.

The global HIV epidemic is extremely diverse, with both similaritiesand differences between regions, countries, provinces and communities .Thereare 34 million people living with HIV/AIDS. However, in order to meet thechallenge of 'Getting to Zero' the global community must work together toachieve universal access toHIV prevention, treatment and care.

Brief History of WORLD AIDS DAY

World AIDS Day was first conceived in August 1987 by James W. Bunn andThomas Netter, two public information officers for the Global Programme on AIDSat the World Health Organization inGeneva, Switzerland. Bunnand Netter took their idea to Dr. Jonathan Mann, Director of the GlobalProgramme on AIDS (now known as UNAIDS). Dr. Mann liked the concept, approvedit, and agreed with the recommendation that the first observance of World AIDSDay should be 1 December, 1988.

Bunn, a broadcast journalist on a leave-of-absence from his reportingduties at K PIX-TV in San Francisco,recommended the date of 1 December believing it would maximize coverage bywestern news media. Since 1988 was an election year in the U.S., Bunnsuggested that media outlets would be weary of their post-election coverage andeager to find a fresh story to cover. 1. What is autism?

Autism isconsidered a spectrum disorder because symptoms and severity vary fromindividual to individual. Autistic children express differently from each other.Autism is characterized by failure of the child to develop communicativelanguage or form proper social communication. But they show motor and otherskills far beyond that of a mentally retarded child.

Autism is characterized by varyingdegrees of impairment to develop communicative language or formproper social communication. Some of them are obsessively pre occupied withinanimate objects such as lights, running water or spinning objects, and alsoby repetitive behaviors. Symptomsrange from mild to severe. Some of them gradually develop extreme talents such as inmathematics and these are retained mostly throughout the life. Studies ofpeople with autism have found abnormalities in several regions of the brainwhich suggest that autism results from a disruption of early fetal braindevelopment

There is a milder form of the disorder known as Asperger'sSyndrome. Other developmental disorders that fall under the Autism SpectrumDisorders are Rett’s Syndrome, PDD-NOS (Pervasive Developmental Disorder, NotOtherwise Specified), and Childhood Disintegrative Disorder. 2. What is meant by “On the spectrum”?
Autismis considered a spectrum disorder because symptoms and severity vary fromindividual to individual. Though the Autistic children show similar traits theyare individual and express differently from each other. one child may have notdeveloped speech and have difficulty in learning how to read and write, whereas another child can easily read and write and can be attending normal school.Some children may display hypersensitivity to touch noise and other may have nosensory issues at all.

3. How common is autism?
According to the survey by Centers for DiseaseControl, autism affects as many as 1 in every 100 children and it is rising by10-15% every year. It is the most prevalent developmental disorder in today’stime. Boys are more susceptible than girls to develop Autism but it is seenthat girls manifest a more severe form of the disorder that the boys.

4. How do I know my child is Autistic?

No two children with autism are alike, each one managing a differentset of symptoms. But there are some signs and symptoms that are common and maybe recognized as early as the toddler years, or even sooner. They usually donot have or do not sustain eye contact and have trouble communicating withother people or they may communicate in ways that others cannot easilyunderstand. They may hardly speak and even if they speak it’s just babbling andnot coherent. They're also prone to repetitive behaviors like flapping theirhands constantly or uttering the same phrase over and over again. They may alsobe over sensitive to sights, sounds and touch.

Criteria for Autism

A total Of 6 or more items of the following with at least 2 from[1], and one each from [2] and [3] are satisfied by the patient:

Failure to develop peer relationships

A lack of spontaneity in sharing interests

Lack of social or emotional reciprocity

5. How did my child develop autism?
No one is yet able to point to a probable cause.Though a disorder as common as autism would have a known cause, in many waysit's still quite mysteriously not known. Recent studies have suggested a stronggenetic basis for autism. There are about 20 sets of genes that may play a partin the development of autism. Genes are not the only one responsible for allthe cases; there are other triggers which are yet not discovered.

6. Does vaccination cause autism ?
Many studies have been conducted to determine if alink exists between vaccination and increased prevalence of autism, withparticular attention to the measles-mumps-rubella (MMR) vaccine and thosecontaining thimerosal. These studies have not found a link between vaccines andautism It remains possible that, in cases, immunization might trigger the onsetof autism symptoms in a child with an underlying medical or genetic condition.

Introduction: Insomnia, this word signifies want of sleep and is used popularly to indicate any impairment in its duration, depth or restorative properties. Insomnia is a sleeping disorder characterized by persistent difficulty in falling asleep.

Normally 7-8 hours of sleep is necessary for human beings. Insomnia can affect not only your energy level and mood, but also it will affect your health.

Types of Insomnia

Primary Insomnia: The term primary insomnia should be reserved for those persons who throughout their lives have never enjoyed restful slumber and in whom none of the usual symptoms of nervousness, depression, or other psychiatric or medical diseases can be elicited.

• Insomnia secondary to another disease condition: Severe insomnia is a frequent complaint of patients suffering from psychiatric illnesses or neurological disorders, brain lesions or history of traumatic brain injury.

Cause

• Certain drugs, medications, caffeine may cause insomnia.

• Fear, anxiety, domestic and business worries which keeps the patient's mind in turmoil can cause insomnia.

• Shift duties and jetlag may cause insomnia. Jet lag is seen in people who travel through multiple time zones, as the time relative to the rising and setting of the sun no longer coincides with the body's internal concept of it. • Difficulty falling asleep at night.
• Waking up during the night.

Introduction: Warts are epidermal outgrowths (skin outgrowths) caused due to infection of the skin and mucous membranes with HPV (human papilloma virus). They are contagious in nature. They can affect any part of the body. They are non-cancerous.

Types of warts
Verruca plana or juvenile warts: They are seen in children usually on the back of the hands or on the face. They are smooth, small, flat topped papules of skin color.

Verruca vulgaris: These affect any part of the body especially the hands and feet. The surface is rough and they are firm in consistency and non tender.

Plantar and palmer warts: They occur on the soles of the feet or on the palms. They are flat due to pressure. They have a tendency to grow inward.

Genital warts: They occur in genital areas of sexually active adults of either sex.

Cause:
• A person can get common warts through direct contact with the human papillomavirus (HPV). One can also get the wart virus by touching a towel or object used by someone who has the virus. People who are more susceptible are more prone to get them.
• Warts usually spread through breaks in your skin, such as a hangnail or scrape. Biting nails can also cause the warts to spread on your fingertips and around your nails.

Complications:
New warts can appear quickly after old ones go away as warts shed HPV virus.

Homeopathic medicines given internally give wonderful results and it is advisable not to cut off the wart. Homeopathy believes that whenever there is a derangement of the vital force or life force or in other words slight imbalance in the body it is expressed through skin in the form of some eruptions or warts.

Most people with high blood pressure have no symptoms at all. There are no specific warning signs. The only way to find out if you have high blood pressure is through regular monitoring.

What is Blood pressure?

Blood pressure is the force exerted on the walls of your blood vessels (Artery) as blood flows through them.

Your heart is like a pump. When it contracts or beats it sends a surge of blood through the blood vessels and the pressure increases .This is called the systolic pressure.

When your heart relaxes between beats, your blood pressure decreases .This is called the diastolic pressure.

Normal blood pressure falls within a range – it is not a set of numbers. But if you’re an adult ,it should be less then 140/90 mm of Hg.If you have Diabetes ,renal insufficiency or heart failure ,your doctor will want it to be not higher then 130 /85 mm of Hg.If your blood pressure goes above this threshold and stays there ,you have high blood pressure.

What does High blood pressure do to your body?

High blood pressure adds to the workload of the heart and arteries. The heart must pump with more force, and the arteries must carry blood that is moving under pressure. If high blood pressure continues for a longtime, the heart and the arteries may not function well as they should and other organs of the body may also get affected.

There is an increased risk of Stroke, heart failure, kidney failure, heart attack.

Blood pressure measurement

When a doctor takes your blood pressure, he or she measures both your systolic and diastolic pressures and records it in numbers. For e.g. if your blood pressure reading is 124/72 ( 124 over 72) ,then your systolic pressure is 124 and your diastolic pressure is 72.The numbers are calculated in millimeters of mercury and recorded as 124/72 mm of Hg.

Chronic Fatigue Syndrome

Introduction: It is a syndrome of severe fatigue that doesn’t improve with bed rest and may worsen with physical or mental activity. Good events such as a wedding, birth of a baby, a long tour or vacation may make you feel tired and exhausted, but the fatigue and tiredness goes away after rest.

Also bad events such as illness of a loved one, death, arguments, disappointed love can make you feel tired, sad, anxious and nervous. But these feelings are temporary and the person is back to normal as time heals most of the wounds.

But—if your feelings of anxiety, depression, or fatigue severely disrupt your daily life or persist for more than a few weeks, or if your feelings seem out of proportion to the event that caused them—or if there is no specific cause—then it's time to see your doctor.

Women are diagnosed with chronic fatigue syndrome far more often than men. Tiredness is severe enough to decrease our ability to participate in daily activities.

Cause: The exact cause is not known. Some of the possible causes are:
• Anemia

• Hypoglycemia

• It is more common in individuals who have previous psychiatric illness.

• Encephalitic illness

• Endocrine dysfunction

• Viral infections, such as Epstein-Barr virus or human herpes virus

• Depression

History of Homeopathy

Homoeopathy was discovered in Germany 200 years back by Dr. Samuel Hahnemann.

He was a M.D physician. However, he was getting disenchanted with the kind of treatment doctors gave patients in his days. Most of it involved purging, one way or another. Patients were given laxatives, emetics or were made to bleed by applying leeches, in an attempt to cure diseases. Hahnemann gave up his practice of medicine and worked as a chemist.

He was also a scholar fluent in 14 languages. So he started translating the original medical work while he was translating one of the work of a doctor he came across the detail that the bark of the Cinchona tree could cure malaria.

Having a scientific mind, he decided to experiment. He ate a little of Cinchona bark and noted it produced symptoms similar to malaria. He gave the same bark to few of his friends and relatives and they produced malaria like symptoms. But he went one step further whenever a patient came with malaria; administration of this bark cured the disease condition!

He continued experimenting with different medicinal substances. He noticed that those that produced symptoms similar to a disease were capable of treating the disease. He formulated the law: SIMILIA SIMILIBUS CURENTUR. (LIKE Cures LIKE).

What is Homeopathy?
Homeopathy is a specialized system of medicine developed by “Dr.Samuel Christian Frederic Hahnemann” based on the natural law of healing “Similia Similibus Curentar” which means likes are cured by likes.

“Homois” means like (Or similar) and “pathos” means suffering.Thus Homeopathy is a system of treating diseases by the administration of remedies which possess power of producing similar sufferings in a healthy being.

There are several symptoms in the diseases of the spine. The person may experience stiffness, limitation in the movement and deformity but pain gets the foremost importance because of discomfort and the disability that is experienced by the suffering person.

The vertebrae are the bones that protect the spinal cord. Each vertebra has a thick drum-shaped area in front called a vertebral body. Between the vertebrae are spaces that allow nerves (nerve roots) to go from the spinal cord to other parts of the body.

What is anterolisthesis?In anterolisthesis the upper vertebral body is positioned abnormally compared to the vertebral body below it. More specifically the upper vertebral body slips forward on the one below. The slippage often occurs as a result of bone fractures

The amount of slippage is graded on a scale from 1 to 4. Grade 1 is mild (20% slippage), while grade 4 is severe (100% slippage).

Symptoms
Symptoms can vary depending on whether the slippage is enough to compress the nerve roots (the portion of the nerve that leaves the spinal cord to connect to other parts of the body) that go from the spinal cord to other parts of the body. In this case, the symptoms will differ depending on where the affected area is.

Symptoms can involve movement or problems feeling sensations (such as heat, cold, pain, etc ), loss of control of bowels or bladder, pain and poor posture.

DiagnosisA doctor will take the patient's medical history and do a physical exam, including checking the patient's reflexes.
X-rays and magnetic resonance imaging (MRI) will also show the slippage of the vertebrae.

Renal Calculi (Kidney Stone) & Role of Homeopathy

Introduction

Kidneys are the organs which help the body get rid of its waste products. These waste products are thrown out of the body through the urine. The waste products consist of various toxic substances, as well as some normal salts and minerals (which are present in excess in the body).

Normally, these salts pass easily through the urine. But, in certain conditions, the salts get deposited in the urinary tract, and this leads to the formation of a stone. Renal calculi are more prevalent in males than in females.

Definition
“Calculus” means a stone. Renal calculi refer to the formation of stones in the kidneys, which mainly consist of the aggregates of crystals of the normal constituents of urine.

Causes and risk factors
The causes can be grouped into different categories for a better understanding, as follows:

Environmental and dietary: where due to any of the these factors, there is increased excretion of the normal mineral salts present in the urine
• Decrease in the urine volume when the environmental temperature is high or the fluid intake of the patient is reduced.

• When a person consumes diet containing higher concentration of proteins, sodium (salt), or reduced concentration of calcium.

• Increased excretion of salts in the urine.

• Decreased concentration of citrates (a constituent of urine, which normally keeps calcium in solution).

Other medical conditions:
• Hyperparathyroidism (increased functioning of the parathyroid gland), which leads to increased calcium excretion in the urine, increasing the chances of stone formation.

• Ileal disease (a part of the intestine), results in increased absorption and excretion of oxalate.

Inherited conditions:
• Genetic factors also play a role in certain people.

According to the main mineral constituent of the stone, they can be of various types: oxalate calculus, phosphate calculi, uric acid and urate calculi, cystine calculi, and xanthine calculi.

Also, depending on the position of the stones in the urinary tract, they can be termed as kidney stones, bladder stones or ureteric stones.

Risk factors
• Persons with vitamin A deficiency are at a higher risk of developing kidney stone.

• Dehydration leads to increased precipitation of the salts in the kidney, hence increasing the chances of stone.

• Patients having recurrent urinary infections.

• Patients, who are on complete bed rest, are at a higher risk.

However, in developed countries, mostly, calculi are seen in healthy young people without any single predisposing factor or any abnormality in the investigations.

Screening and diagnosis
• In most of the patients, the presence of a stone is not known for a long period, unless it produces any symptoms or it is accidentally diagnosed during other investigations. Most of the stones remain silent.

• The majority of the symptoms are produced either due to the passage of the stone through the ureters (narrow tubes carrying urine from the kidney to the bladder), or due to a secondary bacterial infection.

• Pain is the leading symptom.

• In some cases, there may be a fixed renal pain in the loins, which may be aggravated by movement or while climbing the stairs.

• In majority of the cases, the patient who is previously not having any symptoms, suddenly becomes aware of the sever pain, which develops in the loins and radiates from the flanks to the groins.

• The pain starts increasing steadily and then reaches its peak within a few minutes.

• The patient may experience this intense pain for 2 hours, after which it may subside or may continue for hours or days.

• During the attack, the patient becomes anxious. He may have profuse sweating and vomiting.

• There may be an increased frequency of urination, dysuria (painful urination), strangury (painful passage of a few drops of urine), or hematuria (passage of bloody urine) during the attack.

• After the attack of renal colic subsides, there may remain a residual intermittent dull pain in the loins or back.

• In case the patient develops urinary tract infection, he may present with fever, chills, burning urination etc.

• Very rarely, the patient may develop complications like anuria (complete absence of urine in 24 hours), due to the obstruction of the ureters.

• Some other complications like severe infection or impaction of the stone in the ureters may occur producing severe constant pain.

Diagnosis
• Diagnosis can be easily made based on the history obtained from the patient. But, a few investigations may be advised to confirm the diagnosis.

• Urine examination: It is a very common test routinely advised for the confirmation of diagnosis of renal calculus. It may show the presence of amorphous material, or blood, or pus in case of a stone.

• Radiological examination: Opaque stones can be seen (phosphate calculi, cystine calculi).

• Excretion urography: Here, a dye is injected in the kidneys and its excretion is recorded at specific intervals after injecting the dye, with the help of a ultrasonography.This helps to find out the non-opaque stones, their position, their size and also the functioning of the other kidney.

Treatment
• Conservative treatment is helpful only in cases of stones less than 0.5cm diameter. It consists mainly of hydrotherapy.

• The treatment aims at dissolving the stone, or aiding the passage of the stone with the urine.

• If the attempts to dissolve the stone fail, surgical intervention may b e advised, which consists of endoscopic or an open surgery.Endoscopic surgery is preferred over the open surgery now a days.

• Another way of dissolving the stones is through extra corporeal shock wave lithotripsy (where, shock waves generated outside the patient’s body are focused on the stone; so that it gets fragmented and can easily pass through the urinary tract.)

• Though, any of the means stated above may be helpful in eliminating the stone, there is every possibility of its recurrence.

Homoeopathic treatment
• In homoeopathy, acute remedies prescribed during an attack of renal colic, act very efficiently in relieving the pain of the patient.

• Other symptoms of burning micturition, blood in urine, painful urination, and increased frequency of urination will also be relieved.

• After the attack of the colic has subsided, we may give homoeopathic constitutional medicines based on the patient’s physique and mentality, his nature of previous complaints etc.i.e. Considering the patient as a whole, and not only his complaints of the renal stone.

• This constitutional medicine will remove from the patient, his tendency of formation of stones; and thus prevent recurrence of the stone.

• Thus, homoeopathic remedies can very effectively be used either during an acute attack of colic or during the silent period. It can effectively control the acute symptoms as well as remove the tendency of the stone formation.

Patient care
• During an attack of renal colic, bed rest is the immediate treatment

• Application of warmth to the affected site may also help in reducing the pain.

• The patients, who are diagnosed as having renal stones, are advised to drink at least 2 litres of water per day.

• Restriction of sodium intake in diet (intake of salt) to prevent recurrence of oxalate stones.

• Avoid vitamin D supplements as they increase calcium excretion in the urine.

• Also, increase dietary calcium intake, but avoid calcium supplements in the form of tablets.

ACNE

Introduction:
Acne is the most common problem with which our teens suffer from. It is viewed as a normal physiological reaction of the skin. The commonest type of acne appears during the teenagers and rarely persists into the third decade. Though it is more common in women than in men, the severity is known to be more in men.

Definition:
Acne is one of the self limited disorders of the skin, usually occurring after puberty, due to a reaction of the skin to various triggering factors. In persons prone to acne, when their skin is exposed to any of such agents, the size of the sebaceous glands (glands present in the skin) increases, resulting in increased production of sebum (an oily secretion of the sebaceous glands).

Causes and risk factors:

• The main cause of onset of acne at puberty is due to increased production of androgens (Hormones) in both males and females.

• Cosmetics and certain medications block the hair follicles and hence promote the formation of comedones (which we commonly denote as blackheads or whiteheads), which are the clinical hallmark of acne vulgaris (the variety which is commonly seen in the teens).

• As the comedones are blocked, the sebum has no outlet; it gets accumulated in the comedones and hence becomes prone to infection. Due to infection with yeast and bacteria, the comedones become inflamed and this worsens the condition.

• Friction and trauma lead to rupture of the comedone wall and hence the oily sebum comes out. This adds to the oiliness of the face. Hence, an oilinessacne cycle ensues.

• Much of the times, acne appears before or during menstruation due to the hormonal changes which occurs during that period.

• Fatty food increases the oiliness of the face and hence is one of the causes.

• Mental stress, anxiety may also lead to development of acne.

Screening and diagnosis:

Acne vulgaris:• It is characterized by non inflammatory to mildly inflamed comedones, which are the earliest lesions.

• Such lesions are self limited, though they may progress to form large inflamed nodules (boil –like).

• These nodules may rupture and may produce unhealthy scars.

• Face is the most commonly affected region; though forehead, cheeks, chin, nose and back may also be affected in many people.

Acne rosacea:
• This type of acne is characterized by erythema (flushing), telangiectasis (a lesion formed due to dilatation of the blood vessels in the affected area) and superficial pustules (a small elevated skin lesion filled with pus like matter).

• Comedones are seen rarely in these cases.

• Face is the most commonly affected area. Unlike acne vulgaris; chest and back are not usually involved.

• Subsequently, it may lead to inflammation of the eyes (keratitis, iritis), which may even lead to blindness.

Diagnosis of this condition is clearly based on clinical findings.
Patient care:
• The patients should take enough care of their skin by washing it regularly; but limit the use of soap.

• They should avoid touching the affected skin constantly.

• The use of cosmetics should be limited.

• Avoidance of the agents which flare up the lesions.

• Do not pick at the acne this may lead to further aggravation and scarring of the skin.

Bird flu (avian influenza)

Introduction

Bird flu has led to the deaths of hundreds of millions of wild and domestic birds and to a small number of human deaths. However Right now, bird flu remains difficult for humans to contract. Most people who have developed symptoms have had close contact with sick birds. In a few cases, bird flu has passed from one person to another.

A major bird flu outbreak could occur in humans if the virus mutates into a form that can spread more easily from person to person this is a cause of great concern. Earlier the flu pandemic of 1918 and 1919 had claimed millions of lives worldwide.

Signs and symptoms

Illness seems to develop within one to five days of exposure to the virus. The exact incubation period for bird flu in humans is not clear. Sometimes the only indication of the disease is a relatively mild eye infection (conjunctivitis). But more often, signs and symptoms of bird flu resemble those of conventional influenza, including:

Fever
Cough
Sore throat
Muscle aches

People who are affected with the most virulent type of virus strain — H5N1 — may develop life-threatening complications, particularly viral pneumonia and acute respiratory distress (ARDS), the most common cause of bird flu-related deaths.

Causes

How humans get bird flu
Migratory waterfowl, and ducks in particular, carry the viruses that cause bird flu. These birds spread the infection to susceptible species, especially domesticated chickens, turkeys and geese, resulting in severe epidemics that sicken and kill large numbers of birds — sometimes in a single day.

Avian viruses generally don't affect humans, but in 1997, an outbreak of bird flu in Hong Kong infected 18 people, six of whom died. Since then, human cases of bird flu have been reported in Asia, Africa, the Pacific, Europe and the Middle East. Most were traced to contact with infected poultry or surfaces contaminated by sick birds.

Direct bird-to-human transmission

Wild birds shed the virus. Infected migratory waterfowl, the natural carriers of bird flu viruses, shed the virus in their droppings, saliva and nasal secretions. Domestic poultry become infected from contact with these birds or with contaminated water, feed or soil. They may also catch the disease the same way humans contract conventional flu — by inhaling the airborne virus. Heat destroys the virus, but it can survive for extended periods in cool temperatures.

Open-air markets, where eggs and birds are often sold in crowded and unsanitary conditions, are hotbeds of infection and spread the disease into the wider community.

H5N1: In the current epidemic, two influenza subtypes have proved especially dangerous — H7N7, which sickened poultry workers in the Netherlands, and H5N1, which has been responsible for the majority of human and avian deaths in Asia. Of these,

H5N1 is of particular concern for several reasons:
Direct transmission. H5N1 became the first known bird flu strain to jump directly from birds to people when it surfaced in Hong Kong in 1997. It has since infected people in many countries. Other strains have caused illness in humans, but none is as severe as H5N1.

Virulence. The virus is especially lethal, killing close to 100 percent of susceptible birds and more than half of infected people. Birds that do survive can shed the virus for at least 10 days, greatly increasing the flu's spread.

Rapid spread. Since 2003, hundreds of millions of birds have died, a loss that's ecologically and economically devastating. It's also alarming from a public health standpoint — widespread infections among birds may lead to more human disease.

H5N1 mutates quickly and is notorious for grabbing large blocks of genetic code from viruses that infect other species. For that reason, it has particular potential to combine with a human flu virus, creating a new viral strain that spreads rapidly from person to person. Risk factors

The greatest risk factor for bird flu seems to be contact with sick birds or with surfaces contaminated by their feathers, saliva or droppings. The World Health Organization (WHO) has confirmed a handful of cases of limited human-to-human transmission of bird flu. But unless the virus begins to spread more easily among people, infected birds or associated material present the greatest hazard.

The pattern of human transmission remains mysterious. Young children seem especially vulnerable to the virus, although some experts note that children are more likely to have contact with sick birds or to play on ground contaminated with droppings. What's more, people of all ages have contracted and died of bird flu. At this point, too few people have been infected to know all the possible risk factors for bird flu.

When to seek medical advice

See your doctor immediately if you develop flu symptoms, including a fever, cough and body aches, and have recently traveled to a part of the world where bird flu occurs. Be sure to let your doctor know when and where you were traveling and whether you visited any farms or open-air markets.

Complications

Most people with bird flu have signs and symptoms of conventional influenza. Some also develop life-threatening complications such as viral pneumonia and acute respiratory distress syndrome, which causes the air sacs in your lungs to fill with fluid, leading to severe breathing difficulties. More than half the people who have contracted bird flu have died.

A few cases of person-to-person transmission have occurred, but they were limited in scale. Still, some health officials fear that it's just a matter of time before avian viruses figure out a way to spread easily among people.

Treatment

Several bird flu vaccines are in the works, and at least one may be available in early 2007. Although that vaccine seems to protect against the H5N1 strain currently circulating, it's not known whether it will be effective against a mutated form.

Prevention

The international effort to prevent the spread of bird flu focuses on the health of both birds and humans. Measures to help control the virus among domestic poultry include:

Culling.

Surveillance programs
Banned birds. Many countries have banned or restricted the importation of birds and hatching eggs from regions with bird flu epidemics.

Avoid domesticated birds. If possible, avoid rural areas, small farms and especially any close contact with domesticated fowl.

Avoid open-air markets. These can be colorful or dreadful, depending on your tolerance level, but no matter how you see them, they're often breeding grounds for disease.

Wash your hands. This is one of the simplest and best ways to prevent infections of all kinds.

Watch your kids. Keep a careful eye on young children, who are likely to put their hands in their mouths and who may not wash thoroughly.

Steer clear of raw eggs. Because eggshells are often contaminated with bird droppings, avoid mayonnaise, hollandaise sauce, ice cream, and any other foods containing raw or undercooked eggs.

Depression is a medical disorder which affects the thoughts, moods, feelings, behavior of a person it can also affect the physical health. There can be various factors like loss of close people, Stressful life situations, difficulty in adaptation to the surroundings which can trigger depression.

In certain cases depression might occur spontaneously without any specific cause. In some cases the depression can be due to some organic brain diseases or a space occupying lesion (tumor) in the brain.

Signs and symptoms

Cervical spondylitis treated with Homeopathy

A case of 37 years old female patient who came to us with severe pain in the cervical region (neck) .The pain started on the left side and radiates to the arm and the forearm. Along with this the patient also experiences tingling and numbness in the arm. Now the complaints have started on the right arm too.

The complaints are aggravated by

Raising the hand upwards
Lifting heavy things
In Sleep

The complaints are ameliorated by massage and pressure. Besides this there is stiffness of the fingers in the morning which becomes bearable after some movement.

The patient also had lot of hair fall and it falls in bunches. She said the texture of the hair has also changed in the past few years.

She said she has lot of tensions and is anxious about the future. She feels who will take care of them when they grow old .She has a daughter and she is very attached to her she becomes anxious if she goes out or doesn’t come home on time. There are lot of negative thoughts about accident and marked fear of death.

Due to this anxiety she saves her money and if her husband spends money she becomes very angry. Due to this the relation with her in-laws are strained as her father in law spends lot of money which she doesn’t like at all even though he earns his own money.

Relations with her husband are very good but since the last 3-4 years she feels neglected in the family. All these things she cannot express it to anybody so there is lot of internal anger. She feels since marriage she has become very sad.

Chikungunya - Role of Homeopathy

The name “Chikungunya” is derived from the makonde word, which means, "that which bends up” this is due to the fact that the affected person by this disease becomes stooped up as the joints are affected.

Chikungunya is a viral fever, which spreads through mosquito bites from the Aedes aegypti mosquitoes (the yellow fever mosquito). A research carried out by Pasteur institute claims that the virus has undergone mutation which enables it to be transmitted the Aedes alopictus mosquitoes also known as Asian tiger mosquito.

Chikungunya virus (CHIKV) is a member of the genus Alphavirus, in the family Togaviridae. CHIKV was first isolated from the blood of a febrile patient in Tanzania in 1953.It has since been identified repeatedly in west, central and southern Africa and many areas of Asia, and has been cited as the cause of numerous human epidemics in those areas since that time.

Incubation Period
The incubation period (time from infection to illness) can be 2-12 days, but is usually 3-7 days. “Silent” CHIKV infections (infections without illness) do occur; but how commonly this happens is not yet known.

Symptoms
The symptoms of Chikungunya include fever, which can last for a few days to few weeks. Sometimes the fever abruptly comes down.

Body temperature can rise up to 102.2 degrees Fahrenheit.

There are skin eruptions are petechial (Bluish red), or maculopapular in nature. The eruptions are mostly on the lower limbs and trunk.

Multiple joints of the body get affected which are extremely painful and this can have debilitating effect on the body. It can also affect the posture of the body.

The other complaints are intense headache; loss of sleep and marked weakness may stay for a period of 5-7 days.

The patient came with the complaints of severe pain in the right shoulder .The complaints had started 3-4 months after being diagnosed with frozen shoulder the patient was put on painkillers and was also taking physiotherapy. Even after taking the medicines and the physiotherapy there was no change in the pain and it was worsening day by day.

The case came to me after they read about the other cases I had treated but there was lot of doubt in the patients mind .The husband had asked many questions about Homeopathy and how it works and will I be able to provide any relief? As we know a correct case history is very important if we have to solve the case I told the same thing to the patient if they co-operate we can provide a solution.

The patient said the pain is unbearable it is pulling and throbbing in character. The pain radiates from the shoulder to the forearm.

She wont sleeps till everyone has returned back even if it is late at night she will wait. There are lots of negative thoughts that come to her mind like they must have met with an accident or something must have gone wrong. There is lot of anticipatory anxiety.

The patient also worries about his son who is 25 years plus and she feels he is too straight forward and how he will deal with the world.

As a child she was very obstinate she wanted things her way otherwise she would be very angry. She is very attached to her family her brothers and sisters. She was very scared of her father but was very much attached to him emotionally. It came as a big shock to her when he passed away she cannot forget this event and this depresses her sometimes. When asked what thoughts come she said she is afraid of the diseases.

The relationship with her husband is very good. There are no problems on this front. The patient also has the habit of preserving things even old things, which are of no use she will preserve for years together. Also she cannot spend her money easily.

During the case taking the patient was so anxious that she asked me many times will she get alright? The intensity of the anxiety was very strong. On observation there was swelling under the eyes, pigmentation on the face was present.

At the end of this she said I forgot to tell one thing that she is very scared of snakes and also gets dreams of snakes. So from the above history her remedy was very clear.

Lichen planus – treated with Classical Homeopathy

The patient came with the complaints of eruptions on the legs ,waist and on the soles.The complaints had started two and a half years back.The eruptions first appeared on the right leg and then have spread to the waist and the soles.The eruptions are very painful and there is lot of scaling of the skin.

Failure to thrive (FTT)-Treated with Homeopathy

A two and a half years old girl was brought to our consulting room for the complaints of very low appetite ,improper development in hieght and weight.The child weighed 9 kilograms and the hieght was 80cms.The child has no other major health issues but this has become a cause of concern as nothing has seemed to work to improve the above complaints.

The child is a hyperactive child she cannot sit in one place even for a short period of time.if she wants something she wont compromise on it she will throw temper tantrums if she doesnt get what she wants.There are also breath holding spells during certains episodes and she turns blue (Cyanosis).She is very obstinate by nature.

When she is angry she will shout at the top of her voice she will throw things.If someone consoles her during this period she will feel much better .She is very poccessive about her things she wont give her things or share them with her friends or her elder sister.

She loves to dance and listen to music.she is an extrovert will mix easily with people .One of the other characteristics was that she was absolutely fearless there is no fear of anything.

In the family history the Father had a cleft palate and Chronic otitis media.Mother has no major illnesses.Grandmother (maternal ) passed away due to Cancer.Besides this there is no other information as far as the family history is concerned.

She was a unplanned child but the mother and father were very happy when they came to know about the same.There were no major tensions during the pregnancy.It was a Full term normal delivary.

A Case of Astrocytoma Grade 4

This case came to me when the disease had progressed so much from where there was no chance of recovery. But I had to at least relieve the patient of his sufferings to whatever extent it was possible so I took up the case.

Ill tell what I saw when I went to see the patient. It was a picture of total calm the patient was laying on his bed. There was a vacant expression on the face and there was no expression of pain as I had anticipated .His pupils were not reacting to light he looked sleepy and drowsy. This expression left a deep impression on me and I made the necessary notes and went on with the case taking.

The patient had undergone a craniotomy and the tumor in the cerebrum in the left temporal lobe was partially removed. The patient was responding well after the surgery. Then the patient had to undergo the radiations as advised by the oncologist but the patient was strongly against undergoing the same. But he had no other option as family members and everyone convinced him about the same.

After completion of the 20th radiation the patient started deteriorating. First the right hand right leg got paralyzed. And the paralysis was gradually developing. There was also complete loss of voice. This happened within a span of 5 month after the surgery. On repeat C.T scan there was a large hyper dense lesion in the left temporo-parietal region.

The other complaints were severe constipation and stools had to be mechanically removed and they were like small balls. The stools were black in color. There was great difficulty while passing them. Also there was complete loss of appetite. There was a delusion that people are talking loudly and fighting.

Earlier there was severe headache when the lesion was detected for the first time with projectile vomiting. But now the patient is calm, doesn’t say anything, and there was a vacant feeling on the face.

The patient came from a small village in Maharastra India .He was an agriculturist and had a big farm. By nature he was very industrious in his village he had brought the latest things in agriculture .He also advised the other small time farmers what they have to do to reap a good harvest. He used to help them and was very compassionate by nature.

During the famine in their village for 3 consecutive years he helped most of the people as he had 4 wells that had water even during the famine and was the main source of water to the people in the village and their animals. He incurred financial loses but felt it was his duty to help them as they had no other option.

Sometimes he used to be sad when he thought about the famine and used to sit alone and think about the situation. When his cow died he felt a big shock and also his son was also very ill. These things affected him a lot as per the family members.

He was quite by nature didn’t talk much but was very intelligent he had learned most of the things right from repairing a stove to repairing the complex electrical motors. He provided his expertise without charging a penny. Financially they were very good.

He was the appointed head of his village and the government had given him special powers because of his honesty and helping nature towards the poor people.

The other thing that was told was that whenever he got angry he would just keep quite, stay alone and if someone consoled him he felt better. There was no revengefulness and he forgave the person easily.

During childhood he was a quite child very attached to his mother. Father was very strict but he was very attached to him. His father was an educationist and had started many schools at the district level. His childhood was good and he was very good in his studies he used to stand first in the class. He had a problem of stammering during the childhood.

His married life was also very good. Good relation with his wife and children. His mother passed away immediately after his marriage when he heard the news of her passing away in an accident he became unconscious and then for 1 month he developed headaches and giddiness. His father passed away due to old age. This also affected him a lot.

He was very particular about his work whatever task he took he completed it properly and in time. He didn’t like to waste time and expected the same from others.

The above details were taken from the family members.

Computerized tomography scan (C.T.Scan) before surgery
A large predominantly non enhancing lesion showing necrosis and measuring about 38x 36 mm with small foci of enhancement and ring enhancement seen in the left temporal lobe. The lesion is seen extending upwards to the fronto-parietal region and protruding through the bone defect associated with gross edema and displacing the structure to the left side. There appears to be no subfalcine extension except due to edema. No evidence of calcification or hemorrhage is noted.

Posterior fossa: Cerebellum appears normal .4th ventricle appear normal

Supratentorial: Cerebral parenchyma on the right side grossly appears normal. Basal cisterns are normal.

Cortical cisterns show obliteration on the left side and effacement on the right side.3rd ventricle and lateral ventricle shows pressure effects. Basal ganglion on the right side is normal and on the left side shows involvement with edema. Midline shift of 12mm towards right is seen.

Imp: 1st possibility: Anaplastic Astrocytoma
2nd possibility Glioblastoma multiforme

C.T.Scan
A large hyper dense lesion is seen in the left temporo-parietal region showing minimal vasogenic white matter edema causing effacement of the ventricles on the same side and minimal shift of the midline structures to the opposite side showing peripheral contrast enhancement and central necrosis.

The patient was not responding to anything there was a vacant look on the face.

I was informed about the above condition when it was too late.

Learning

The homeopathic medicines are deep acting remedies as we saw how even in this pathologically advanced case when the laxatives and other medicines where unable to give relief to the patient .The correct homeopathic remedy helped the person to get relief.

We can achieve at least palliation in its right term and relieve the suffering of the terminally ill patients.

Uterine fibromyoma (Fibroid)

It is commonly known as fibroid but the correct terminology should be fibromyoma or myoma .As it develops from the muscular layer of the uterus. The fibroid is an admixture of muscle tissue and fibrous tissue. It is benign in nature.

Cause

Heredity as it runs in families heredity can be one of the causes for the formation of fibroid.
It can also occur during the reproductive period due to increased blood supply to the uterus.
It also depends on the hormone estrogen for its growth.
The associated factors along with fibroid are endometrial hyperplasia and infertility.

IncidenceThis is the commonest type of neoplasm in a women.Negroes get fibroid 3 times more then the whites.

Location of the fibroids
In order to understand the location of the fibroid in the uterus let us understand the structure of the uterus in brief. It is a internal genital organ also known as the womb.
It is situated in the center of the pelvic cavity.

Parts of the uterus
It is divisible in two parts by the slight constriction at the level of the internal os.

a) Corpus or Body: The corpus is further divided into fundus the part which lies above the openings of the uterine tubes (Where the fallopian tubes are attached refer the diagram).The body proper is triangular and lies between the opening of the fallopian tubes and the isthmus.

b) Isthmus: it is the constricted part which measures about 0.5 cm and is situated between the body and the cervix.

c) Cervix: it is the lower most part of the uterus which is almost cylindrical in shape and a part of it is incorporated within the vagina. It is divided in to the supravaginal part which lies above the vagina and the vaginal part which lies within the vagina.The vaginal portion is also known as portio vaginalis .There is an opening at the lower end of the portio vaginalis which is known as the external os.

In women who have not given birth the vaginal part of the cervix is conical in shape and the external os looks circular. In women who have given birth to a child it is cylindrical in shape and the external os has bilateral slits which is caused due to the tear of the circular muscles surrounding the external os during child birth and it gives rise to anterior and posterior lip of the cervix.

Cavity: If the cavity of the uterus is studied in sections; in the saggital section the cavity of the corpus is narrow slit but on coronal section it is triangular in shape and the uppermost part is called the base this communicates into the lumen of the fallopian tubes on either sides.

The cervical canal is spindle shaped which opens above in the uterine cavity through the internal os and below into the vagina through the external os. The length of the uterine cavity from the external os to the fundus is about 6.5 cm of which 2.5cm is the length of the cervical canal.

d) The Fallopian tubesEither of a pair of tubes extending from the uterus to the paired ovaries in the human female also called oviducts, technically known as the uterine tube. At one end the long, slender fallopian tube opens into the uterus; the other end expands into a funnel shape near the ovary. The epithelium that lines the tube is covered with cilia that beat continuously toward the uterus

The Fibroid of the uterus maybe present in the Body of the uterus that is known as Corporeal this is the commonest type. The other type is found in the cervix of the uterus.

Types
Interstitial ( Intramural) : It grows in the substance of the uterine wall and may remain as such in especially when they are small or maybe sub mucus or subserous.The posterior wall of the uterus is the commonest site for the development of the fibroid.

Subserous (Sub peritoneal): The fibroid lies underneath the peritoneum (Peritoneum is the serous membrane that lines the abdominal and uterine cavity) and it can be sessile or pedunculated.If it grows between the layers of the broad ligament of the uterus then it develops ligamentory fibroids.

Submucous: mostly it is a single fibroid which grows beneath the mucous membranes of the uterus and projects itself in the uterine cavity. It maybe sessile or pedunculated in character. Due to the expulsive force of the uterus the sessile growth can change its character. The following things can happen to a sub mucus fibroid.

a) Formation of sub mucus fibroid polypus
b) It can become infected with pus formation and erosion of the same.
c) This is the commonest variety which can undergo malignant changes
d) It may rarely cause inversion of the uterus due to the expulsive force of the uterus.

Clinical features of corporeal fibromyoma

Occurrence: Uterine fibroid arises during the menarche (beginning of the menses) till menopause. During the age of 35-45 fibroids are more evident. After menopause the fibroids shrink and never develops freshly this occurs due to the hormonal changes that take place in the body after menopause. Sometimes the menopause can be delayed due to the fibroids

Symptoms
Most of the women suffering from fibroid are symptomless.The following symptoms can be seen in some of the patients.

Menorrhagia: excessive bleeding during the menstrual cycle this is the commonest symptom which often becomes a regular feature with passage of clots.

Metorrhagia: irregular bleeding is a feature of submucous fibroid with ulceration or rarely endometrial carcinoma.

Dysmenorrhoea: Pain in the abdomen and lower back can also be noticed. The pain is unbearable in nature.

Leucorrhea: This is the white discharge and it occurs particularly if there is a fibroid polyp.
Abdominal lump the woman comes with the complaint that she feels a lump in the abdomen. The lump has to be big enough which corresponds to 14 weeks of pregnancy to be palpable.
Infertility about 30 % of the females can suffer from infertility due to uterine fibroids.
Pressure symptoms like frequency of urination or urgency to urinate as a result of the pressure of the fibroid on the bladder.

Anemia: it can develop due to the excessive bleeding but it is a gradual process. The patient may suffer from palpitation and weakness.

Pain this is a rare symptoms .pain can develop due to the following reasons.
a) If a fibroid polyp is being expulsed
b) Inflammation
c) Derangement of the blood supply to the fibroid.
d) Twisting of the Subserous fibroid

SignsWhen there is excessive bleeding anemia can be present

On examination of the abdomen there is a feeling of a lump. The surface of the lump maybe smooth .There is dullness on percussion of the area.(A type of Physical examination).
Bimanual examination confirms that the uterus is enlarged in size. Then the uterine fibroid is carefully assessed for red degeneration, infection and rarely for sarcoma.

Treatment

Homeopathic treatment: After complete case study the patient is put on the constitutional treatment. This should take care of the problem. The patient is kept under observation and is checked every 6 months for the progress in the treatment

Conservative treatmentIf the bleeding is excessive and the patient is in a bad state then blood transfusion will be helpful. Treatment of the anemia.

Cervical fibromyomaThese are not the common type of fibroids .They contribute 1-2 % of all types of the uterine fibroids.

LocationIt can be located at the portio vaginalis (Please refer to the image) and it can project in the vagina and become polypoidal.

At the supravaginal cervix the fibroid can be situated in the following positions
Anteriorly it pushes the urinary bladder up

Posterior it obliterated the pouch of Douglas and can cause pressure on the rectum.
Lateral in the broad ligament due to this the ureter is pushed laterally but it remains below the fibroid.

Central it causes expansion of the cervix from all the sides.

SymptomsIt doesn’t cause any menstrual disorder but it causes pressure symptoms like frequency of urination, defecation on straining, Constipation.It can also cause problems during child birth.

SignsOn examination of the abdomen the uterus and its parts are felt separate being situated on the top of a firm pelvis mass.

Examination through vagina (Per vaginum)The cervix of the uterus is felt expanded due to the filling up by the tumor. A mass can be felt which is fixed to the cervix which is pushed high up behind the symphysis pubis.
The feature of the mass is its firmness, fixity, pelvic position and the attachment to the cervix. The body of the uterus is felt separate from the mass.Examination through the rectum under anesthesia and sounding the cervical canal are essential steps for diagnosis.

Menopause and Climacteric

This is a phase in women’s life when there is final cessation of menstruation which is known as menopause. This is the period when the women changes from the reproductive life to one of senescence. This happens due to cessation of ovarian follicular function. It is a natural physiological process.

Menopause occurs due to exhaustion of eggs from ovarian follicles and the resultant hormonal changes that happen. (Estrogen deprivation)

Changes that take place in Climacteric or Menopause and Post menopausal age
There is progressive atrophy of genital organs with more deposition of the fibrous tissue in them. The ovaries become small with furrowed surface. The ovarian vessels become sclerosed.There is also shrinkage in the fallopian tubes. The uterus becomes small which occurs due to shrinkage of the muscles .Cervix atrophies and the cervical secretion becomes scanty thick, and later disappears. The lining of the vagina atrophies with loss of rugosity.The vagina contracts with shallowness of the fornicis.Vulva also atrophies with narrowing of the introitus.The cellular tissues of the pelvis become lax.

Changes that take place in the secondary sex characteristics

Breasts: They show gradual atrophy of the glandular tissue which results in flabbiness. They tend to become pendulous due the fat deposition around them.

Pubic and axillary hair becomes sparse.

Physically the body weight decreases after 65 years of age. The skin develops wrinkles, becomes less elastic. There is subcutaneous fat deposition in the hip and the thighs. There can be changes in the spinal column due to osteoporosis. Due to the hormonal changes that take place women tend to suffer from osteoporosis.

During menopause some changes in the digestive system are seen. This can cause indigestion, constipation, hypochlorhydria (decreased secretion of acids).

Psychosexual: the urge for sex is increased at menopause, the person tends to be more sensitive and emotional upsets are common. As an aging process the urge for sex wanes after 60 years of age.

The timing of menopause

The age at which menopause occurs varies widely from 40 years of age to 55 years with the mean age being 47years.The other factors that influence menopause is the genetic makeup,race and climate. Menopause is earlier in women who stay in the tropics as compared to the women who stay in the colder countries.

If the menopause occurs before the age of 35 it is considered early and if it sets in after the age of 55 years it is considered late.

Early menopause can occur due to ovarian failure, oophorectomy or irradiation. Where as delayed menopause is due to some pelvic pathology like a uterine fibroid or in association with diseases like diabetes mellitus.

Clinical feature of Menopause and climacteric
Menstrual Symptoms

The flow during menstruation becomes scanty progressively which is followed by their cessation.

Menses occur after prolonged intervals and finally ceases.

Sudden cessation of the menses. Any excessive bleeding or irregular bleeding is not due to menopause as it is commonly believed by the lay person. This can be due to some pelvic pathology.

Most women do not experience other symptoms they adapt nicely to these changes that take place in the body during menopause. Few women have mild symptoms of weight gain, pain in the joints, and increase in the sexual desire with gradual loss for the same.

SignsIn normal women the following signs occur during menopause and thereafter.

Genital signs
Vulva: There is gradual narrowing of the vaginal introitus.There is progressive atrophy of the vulva and the hair becomes scanty.

Vagina: there is thinning of the mucus membrane and loss of rugae. It becomes narrow with tenting of the vaginal vault.

Cervix: Portio vaginalis atrophies and gets flushed with vaginal vault.

Uterus: it becomes small and hard

Adnexae: Ovaries become small and impalpable.

Diagnosis: the clinical features plus the atrophic vaginal smear and elevated plasma FSH levels are considered in diagnosing menopause.

This syndrome is prevalent in the young women in the reproductive period. It is a complex disorder characterized by excessive production of hormones known as androgens by the ovaries and the adrenal glands. Due to this the ovarian follicles fail to mature.

Signs and SymptomsIncrease in the body weight (Obesity)

Abnormalities of menstruation. The patient can suffer from scanty menstruation (Oligomenorrhea), Absence of menstruation (Amenorrhea) and infertility. The patient comes with the complaints of irregular menses and says she doesn’t get periods for 3-4 months.

On internal examination bilaterally enlarged cystic ovaries are revealed. Sometimes due to the excess weight the ovaries may not be revealed.

Sonography : transvaginal sonography is especially useful in obese patients
Serum values
Luteinising hormone (LH) levels or the ratio between LH: FSH is > 3:1 (Follicle stimulating hormone. FSH)
Reversible oestradiol: Oestrone ratio. The Oestrone level is markedly low.
Androstenedione is elevated
Serum testosterone and DHEA-S may be marginally elevated.

Laparoscopy bilaterally polycystic ovaries are detected; which is the characteristic feature of PCOS

It is a state in which some women suffer from irritability, anger at trifles, mood swings, lassitude, sleeplessness, headache, nausea, constipation, bloating of the body (edema).Breast become painful, lumpy and tender. 1) Is it safe to take Homeopathic medicines during Pregnancy?

Yes. It is safe to take homeopathic medicines from a qualified homeopathic doctor .If the treatment is planned in a proper way it will have lot of benefit to the mother and the child. The child will be much healthier and the medicines will help to fight the hereditary diseases which are carried from generation to generation. In fact if homeopathy is given during pregnancy the delivery is normal and easier. The child is relatively healthier and balanced emotionally, intellectually and physically.

2) Does one have to take the Homeopathic medicines for a long time?

No. The duration of the treatment depends upon the nature of the disease and the fighting capacity of the body. The treatment is continued for certain time period so as to improve the resistance of the body to fight diseases. The prescription of homoeopathic medicines is based on the similarity of the symptoms of the patient with those produced by the drug (in its purest form) when proved (clinical trials) in healthy individuals. 5) Why do homeopaths do not tell the name of the medicines that have been given?

The other fact is that certain medicines have to be changed and given as per the state of the disease and recovery which is very essential. At the end of the treatment if the patient wants the line of treatment that had been given it can be provided to him but at the end of the treatment.

6) Does homeopathy believe in disease diagnosis and pathological investigations?

Yes it is very essential to diagnose a disease as for the selection of the remedy we require to know the characteristics symptoms and not the disease symptoms. For e.g. If a person is having an attack of asthma he will be having wheezing and congestion of the chest and he gets relief only if he throws his head backwards. The symptom “throwing head backward relieves” is very peculiar which will help us in getting the right remedy. A good knowledge of disease is important. The pathological investigations are also important to know the prognosis and disease diagnosis.

7) Are there any food restrictions during the treatment?

Yes one has to avoid coffee, raw onion and raw garlic. One should also avoid eating mint, camphor and menthol as these can hinder the action of the homeopathic medicine or antidote the effect of the medicine.

8) Does one have to stop other medicines which the patient has been taking?

No. The patient should not stop the medicines which he has been taking. It should be tapered after the patient is improving this should be done in consultation with the other treating physician.

In some cases the other medicine cannot be discontinued in such instances both the medicines have to be continued for the benefit of the patient. The treatment should be planned and properly implemented.

9) Do homeopathic medicines bring about aggravation initially?
In some cases there is an initial aggravation of the symptoms but it is a good sign that the medicine is right and has started acting. But this happens in a few cases not all patients will have an aggravation.

Homoeopathic medicines are also available in liquid form or tinctures, which can be ingested directly or by diluting them in water.

13) Does the Homoeopath give the same white pills for all illnesses/patients?

Certainly not. Although it appears that same white pills are given for all illnesses or patients it is not so, as there are more than 3500 remedies to prescribe from and new medicines are added to the list regularly, after their proving and clinical trials. All homoeopathic medicines are available as liquids with an alcohol base. In order to dispense a medicine to a patient, the indicated remedy in the liquid form is poured on the white sugar pills, which merely act as vehicles to transfer the medicinal property. Hence it appears that the same white pills are administered to all.

14) Is homeopathic treatment effective for children, adults, woman and elderly?

Role of Homeopathy in Autism

Autism is considered a spectrum disorder because symptoms and severity vary from individual to individual. Autistic children express differently from each other.Autism is characterized by failure of the child to develop communicative language or form proper social communication. But they show motor and other skills far beyond that of a mentally retarded child.

Some of them are obsessively pre occupied with inanimate objects such as lights, running water or spinning objects. Some of them gradually develop extreme talents such as in mathematics and these are retained mostly throughout the life.

Studies of people with autism have found abnormal ities in several regions of the brain which suggest that autism results from a disruption of early fetal brain development.Now we come to the point of what homeopathy has to offer to the child so that we can help the child to lead a better life. One of the important facts is that in homeopathy we have these symptoms covered by different remedies and can use the symptoms expressed by the behavior of the child for the selection of the medicine.

It is very important to get a proper history of the patient and the importance of observation comes into play even the minutest detail can help in deciding the remedy. Peculiar symptoms are required so the parents should observe the Childs behavior in details and note them down in a book.

Here I would like to illustrate a case of autism which we treated
A young girl of seven years was brought to us, with the following complaints:

The child would sit in a corner, for hours and days together. She had no interest in food, play or studies. This attitude was there from childhood.When I first saw her in the clinic, I observed that her face was flat & showed no expressions. This child would not mix or play with anybody and always wanted to stay alone. She would sit alone, as if she was in her own world and had nothing to do with the worldly affairs.

She took no interest in her studies. (There was no problem of intelligence, because the IQ was 80+ ) She would not emote to anything, there was no reaction from her even if she was scolded, explained or persuaded

Physical complaints:Constipation could not pass stools for 6-8 days. Stools - hard with lot of pain and discomfort. Severe diarrhea when forced to eat. Perspiration offensive 3+, staining the clothes. Sleep disturbed, would get up a number of times, occasional weeping talking during sleep.

On further detailed study,it was revealed that this child was adopted by the couple, as they did not have any children, after 10 yrs of marriage. The child was bought from an orphanage, and no details were available about her parents or about her background.

Diagnosis : AutismBased on ‘Diagnostic and Statistical Manual of Mental Disorders’ – Fourth Edition by the American Psychiatric Association, Washington, DC, we can classify each of the cases in the following way:A total Of 6 or more items of the following with at least 2 from [1], and one each from [2] and [3] are satisfied by the patient:

A case of Urinary tract infection- Acute case presentation

A male child aged 5 years was brought to the consulting room with high grade fever since 4 days. The fever was not responding to the selected Homeopathic remedy. The patient had always responded very quickly in acute conditions and this was the first time that he was not responding.

As the fever was not responding the parents visited his Pediatrician in the morning but did not start the medication. They brought the child in the evening to the consulting room.

The fever was 103 degrees F the child was feeling cold and he was restless moving from one corner of the room to another. He was complaining of severe headache marked irritability was present. His complaints were aggravated by cold drinks, Fanning and he felt better when he was carried.

When we asked if the parents have observed anything in this course of 4 days the mother said as the fever increases the child starts feeling cold then his abdomen and back are very hot to touch but his Hands and feet are cold during the fever this was a very interesting symptom. There was also burning in the urine.

We had got him investigated
WBC count was high
Malarial parasite was absent
There was severe urine infection.

On examination
There was no neck rigidity present.
Respiratory system was clear
Throat was mildly congested
There was redness at the tip of the penis (external meatus)
Abdominal examination revealed no abnormality.

Patient was given Cantharis 30 2 pills to be repeated every 4 hourly but we asked them to stop the medicine as soon as he felt better. Even after 1 dose if he felt better they had to stop the medicine.

Next day the parents called and told that after giving just 1 dose his fever subsided and is much better. We examined the patient again after a week and he was totally back to normal.

Case of involuntary urination in sleep

A case of a child aged four and a half years old. She suffered from involuntary urination at night in sleep as well as during daytime while sleeping. This was going on since the last 2 years. Her mother was very worried about the problem. There were no other complaints the child suffered from.

The child was very shy and during the whole case taking she didn’t utter a single word. She was just smiling and sat on the sofa next to her mother hiding for sometime. Then she used to peep and smile.

Her mother told that she had a very bad temper in anger she throws things, hits her elder sister. She is very stubborn and throws temper tantrums if she doesn’t get what she wants; she will weep for a longtime and wont listen for quiet sometime.

She is afraid of dogs, cats +3, darkness and being alone. She loves to dress up well, likes lipstick, henna, and nail polish. She loves traveling and is very happy while traveling.

She also has a habit of thumb sucking. Her mouth is always open. There is lot of extra hair growth on the body.

A case of chronic tonsillitis

The patient was brought to me for an opinion weather something could be done for the recurrent throat infections. The patient was advised to undergo tonsillectomy by the local doctor as she was not responding to the treatment. As her mother who was under our treatment for allergies was feeling much better she brought in her daughter.

The patient was an 11years old girl well dressed up and was a bright child. She got along very well with people. She mixes easily with everybody and loves to be in a company. But of late she doesn’t go and talk on her own.

She also has a habit of biting her nails. Her mother told us she was a very good child quiet and satisfied. She gets along very well with her brothers and sister. She is average in her studies and likes to study.

When we asked her if she had any fears? She told she is very much afraid of lightening and thunderstorms+3 since the beginning.

The patient has severe pain in the throat more on the left side which is aggravated by swallowing+2, and eating sour things. There is marked thirst for cold water.

On 18th September her mother came to me as her other daughter who is pregnant came for a check up and she wanted to put her on homeopathy.

And while talking she said doctor do you remember my younger daughter and of course I used to wonder how this patient is doing? To my surprise she told after the medication she has not had a single episode in the last 4 months and her health is very good. I was very happy to know this and then I thought of presenting this case.

In this case the symptoms were so precise and characteristic that it was difficult to miss the remedy. So in cases of children it is very important to start a conversation with the kid. Then the symptoms that are expressed will make it easy to find the right remedy.

A Case of frozen shoulder with wasting of the deltoid muscle

This patient came to me at 11pm at night to my residence with severe jerking pain in the left shoulder with radiation to the arm. He said there is also numbness and heaviness in the hand.

He has been suffering from the above complaints since the past 1 year and he has already taken 3 steroid injections with little relief. The orthopedic has advised him to undergo surgery but the patient cannot afford the cost of surgery.

There was also wasting of the deltoid muscle and the arm. He also experienced weakness in the left arm. This patient by profession is a Banjo player (Drums) so it was a very desperate situation for him to get alright because he was unable to play the drums.

The patient is a dropout from the school and has not bothered to do anything in his life now he is 37 years old but still doesn’t have a steady income. He felt very bad when people asked him what he has done in his life. You have wasted all these years. So now he wants to prove himself and do something in life.

He weeps when he is alone, sometimes also weeps in front of others. Earlier he never used to express himself after his marriage he has started expressing himself.

He becomes very angry and afterwards feels guilty for the same. He controls himself and will try and not to do the same thing again. If someone tries to console him he doesn’t like it at all.

The patient has also attempted to commit suicide by jumping in a well.

The patient is a chronic smoker and alcoholic as far as the exposure is concerned he didn’t divulge the details.

He likes music a lot which also ameliorates his complaints.

This was a brief mental picture that I took as the patient had to go home and his little daughter who accompanied him was very restless and wanted to go home.

His strength has come back and there is no pain and he wanted to discontinue the treatment. As I was confident that the medicine has done its job. I told him to report if any problem arises. He agreed and we stopped the treatment.

It’s been a year and no complaints have reoccurred he is now able to play his drums and perform well.

This was a very interesting case for me as the patient expressed the modality bending backwards ameliorate with such intensity that the Remedy just flashed in the mind Dioscorea.v.After refering to Herings guiding symptoms and analysing the case Dioscoria.v 30 was given.

Case of Eczema

The patient came with the complaints of eczematous eruptions on the body which started after her second pregnancy 18 years back. Since then the patient is suffering and nothing has helped her. She came to me because her daughter got rid of her skin complaints which were also chronic in nature.

The eruptions started on the right leg then on the abdomen and other parts of the body. The patient was worried about the eruptions which were visible to people she felt disgusted about them. There is severe itching with redness which is aggravated at night +2, morning, sun light. She feels better after application of warm water. There is no discharge from the eruptions.

There is also pain in the fingers of both the hands and pain in the right knee which is aggravated in the morning and after working in water. She feels better after movement.

In the life space the patient is born and brought up in Delhi (India).She was the youngest child in the family and was a pampered child. She got whatever she wanted. She was very shy as a child and took lot of time to mix with people. She was average in her studies and was very sensitive emotionally. She used to cry very easily and if someone consoled her she used to feel very good.

Things changed after she got married as she had to shift to Mumbai (Bombay then) this was a big change and till date she is unable to adjust to the environment in Mumbai. She feels very lonely and misses her family a lot. To add to it her husband is a very busy business man and has hardly any time for her. But she says they have a good relationship.

She has 2 children daughter and son. The daughter gave us the following history she said her mother is very worried about her marriage and constantly nags her. She tells her that she is a big burden on the family. She said her mother is also very lethargic and doesn’t like to do the household chores. So the daughter has to do all the cooking .She becomes very angry if someone contradicts her. She has been very partial towards her son who gets whatever he wants and the daughter gets nothing.

Learning from the case was the importance of taking case history from the relatives close ones which can make a difference in getting the right remedy. We also saw the cure taking place according to Herings law of cure complaints disappearing from above downward.

Also one should have a proper case recording system so if the patient comes to you after years together for some other ailment the constitutional medicine will do the job; of course one has to retake the case and confirm the remedy once again before prescribing.

A case of Allergic rhinitis

A case of allergic rhinitis with a past history of bronchial asthma in the childhood. He has been suffering from cold +3, sneezing, lacrymation and watery nasal discharge since the last 6 months. The patient was under allopathic medication earlier which gave him temporary relief.

The sneezing starts suddenly and it is aggravated after Bathing +3, Dust mites, Cold drinks, change of weather. If he takes warm tea he feels relieved for sometime. During this episode he feels cold +3.he has an aversion to fanning. He feels thirsty and takes about a glass of water at a time.

The patient said this was the only thing that was bothering him as he was not getting alright inspite of taking medicines.

We had given him a case history form and had asked to write down all the things in details. He had written down the history in a very methodical way. Currently he is working in his fathers manufacturing unit of liquid fertilizers. He got his bachelors degree in science with Physics as the special subject in the year 2002 and he joined his fathers business as he was interested in it since his childhood.

He says he is happy with his job and informs us that the business is at the peak in the rainy season and very low for the rest of the year. He utilizes his time in learning something new he has been learning accounting in his spare time.

The patient is very mild by nature .When we enquired about his childhood he said it was a very happy childhood, He would love playing in the rain, hang out with his friends. He was average in his studies but he liked to study. He was interested in his fathers business.

He mixed easily with people but never expressed himself when he got angry. He
doesn’t tell anyone if he is angry.

He has a marked fear of darkness and ghost. In darkness he feels that the ghost will come and catch him. He also feels that someone is behind

The he went on to describe his family there are 4 members in his family Father, mother, elder brother and he himself. His father has undergone a difficult life as his father expired when he was just 10 years of age. He faced lot of hardships but he developed his current business and there were lots of ups and downs in it. During this time he developed the habit of drinking and to the date he drinks half a bottle of whisky everyday. He is suffering from hypertension and is under antihypertensive.

Father is very strict and wants things done on time otherwise he becomes angry. His mother used to work in the reserve bank of India and has retired she suffers from Diabetes and is on insulin. His elder brother is working as a systems engineer and is in good health. He shares a good relationship with all of them.
Past history
Bronchial asthma during the attack he used to be relieved by sitting up.

A peaceful death- case of Cancer of Esophagus

It was a case of Ca Esophagus at the O.G. junction with metastasis in the spine. The histopathology report showed Grade 2 adenocarcinoma of the stomach with lymph nodes almost replaced by metastatic deposit. The patient had undergone esophagectomy and gastric pull up surgery in July 2004 along with chemotherapy that was also administered.

He had complaints of difficulty in swallowing solid food and malaena since 9 months along with pain in the upper part of the abdomen and retrosternal region. The patient was 70 years of age during the surgery.

After the treatment there was relief for 6 months after this he developed severe backache and difficulty in swallowing food. For this the patient underwent another surgery and esophageal stenting was done this gave some relief in swallowing. There was a recurrence of the neoplasm at the lower end of the esophagus again with metastasis in the spine. Bone scan showed increased tracer intake at D10 and collapse of the D8 vertebrae with partial lytic sclerosis.

By this time the patient develop very severe backache which didn’t respond to the normal course of treatment so the doctors advised that he should undergo radiation. Along with this another cycle of chemotherapy was started which the patient was unable to withstand. After this he became very weak and bed ridden and there was severe backache. So the treatment was stopped. At this point the family member’s thought of trying out homeopathy.

They asked if we can relieve the pain and anything can be done for the patient. So I had to go for a visit as the patient was not in a position to come to the consulting room. When I went to the patient’s house I saw a frail man lying on the bed with his legs flexed at the knees and drawn up. He was very cheerful and thanked me for coming to see him. He was well orientated and his memory was pretty sharp. He gave me the key note when he said doctor “Iam physically weak but mentally strong”.

Due to the surgery the voice had become very low pitched but he explained his symptoms he said he had severe burning pain in the back and also at the site of the surgery. The complaints were aggravated by sitting he was unable to sit even for 5 minutes, by walking the pain radiates to the left thigh. I asked him why was he sleeping like this with his legs drawn up he said this is the only position that gave him relief.

Pain was the main concern of the patient and he wanted to go out for his walks. There were also external growths on the scalp and one on the retrosternal region. These were painless and didn’t cause any trouble to the patient.

He said till 70 years of his age he has lead a very healthy life he has not suffered from any major illness expect for the Typhoid and Small pox that he suffered as a child. He said he has no worries and is quite satisfied as he has had a good life. He worked for an overseas bank as a chief officer. It was a transferable job but he liked the job very much. He used to work very hard from morning 7am till 11 pm at night. He liked to complete whatever job he took and when he retired he had 700 medical leaves pending. Overall he enjoyed his work.

As a person he is mild by nature and very sensitive if somebody said something wrong he got hurt emotionally and was unable to forget the event. After his illness he weeps very often when something hurts him emotionally.

In childhood he used to mix very easily with people and they stayed in a joint family. He was very much afraid of his father as he was very strict. He had 11 brothers and 1 sister then also he used to feel he has nobody. This was all I could get about his childhood.

Then he said he gets lots of dreams they are as follows

I received a call from the son that his father passed away peacefully. He told me his father told him that he wanted to visit to the consulting room once as he felt obliged as his pain was much better. His son told they were happy as at least the last months were pain free. This was the only request the patient had made when I had gone to see him. He said just get me out of this pain. And when the right Homeopathic remedy was given palliation was achieved.

Understanding of the caseThe patient was given Colocynth as an acute remedy as the acute picture matched with colocynth. Lycopodium was his constitutional remedy but as we know when the susceptibility of the patient is low and advanced pathological changes taken place when the constitutional remedy is given the body sometimes cannot take in the initial aggravation. After the improvement in the pain the patient was given Lycopodium in the LM potency after reviewing the case once again. And the patient responded in a very good way and further improvement was seen.

It should be noted in treating terminally ill patient one has to be very careful in selected the potency and its correct application. We achieved palliation and we were able to relieve the pains of the patient which didn’t subside even after taking painkillers. I remember one case where there were multiple pathological fractures , patient was on morphine and then also the pain was tremendous in this case also the acute picture matched with Colocynth .Colocynth 6 was given and the pain was much better.

So perceiving the case is very important if we have to deliver what we promise.

A case of Grade 1 anterolisthesis of L4-L5

The patient complained of severe throbbing biting pain in the left gluteal region with difficulty in walking since the last four months. The complaints got aggravated after a
fall in the bath room.

The pain is aggravated by walking +2, Standing and the pain I s ameliorated by lying on the painful side and rest.

He MRI –LS spine shows grade 1 anterolisthesis of L4-L5 mild posterior bulge of desiccated L4-5 disc indenting the thecal sac and the left L5 nerve root. The patient comes from a lower economic background and they didn’t have the necessary funds to carry out the surgery that had been advised by the orthopedic.

So the patient was referred to us if we can relieve the pain. So we took the case. The patient comes from a small village .She is very much lively in her talk she said she used to work in the farms since she was young and she enjoyed working in the village. She came to the city of Mumbai after her marriage and here the situation is very different.

They are staying in the slums in a very small room and people around them are not good. She is not happy. Her husband is very dominating and he keeps on commenting about the way she walks due to the pain and insults her often. She feels very bad about it and when insulted she weeps. We asked the husband why he does it he said he had to get married to her against his wish; as in their community what ever parents decide has to be followed.

He is not happy with their marriage so he seldom gets angry on her. He is very dominating and doesn’t allow her to talk even when he was there him the consulting room we had to ask him to wait outside to interact with the patient.

The patient then told us there were lot of problems in their martial life and they didn’t get along well.Inspite of the fact that she looks well after her in laws and brother in law. She is never appreciated for it. She feels very much neglected as there is nobody on her side. Her father passed away due to tuberculosis at a very young age since that time she is leading a very tough life.

She likes to listen to music very much and she feels relaxed after listening to it. She also sings and which helps her to relax. She is very sensitive by nature and if some says something wrong she cannot forget it broods over it for a long time. But she will express it when she is angry. She says she is not revengeful at all.

A case of Claudication, Radiculopathy and spondylolisthesis

An interesting case of claudication and severe pain in the right gluteal region .The patient has turned to homeopathy because even after undergoing an endoscopic surgery on her spine the pain has not yet reduced.

The pain is so severe that she just cannot do anything and has to be in bed most of the time. She is taking pain killers and calcium sprays which are of little help.

There is severe pain in the right gluteal region with numbness .There is pain and uneasiness over the area of L1-L2.Burning sensation in the right leg.

The patient says that she has lost the confidence of lifting anything. She also has hyperacidity and lots of flatulence.

After the initial details the patient told that she was a premature baby.
As a child she suffered from febrile convulsions. And once she had fallen from a height of 10 feet and she had developed a hematoma.These didn’t have any major effect on her.

She told us that she came from a poor family and it was a big family with 3 elder brothers and a younger sister. She was an unwanted child and her mother had refused to accept her she was even deprived of breast feeding .Now the question that arose in our mind was why the mother did refuse to accept her own child? She told us that in their tradition if after the birth of 3 male children if the 4th child is a female it is considered to be a great misfortune for the family.

Her mother didn’t allow her to go to school till the age of 9 years. But she had a very strong desire to study. As she was her father’s pet she managed to get into school. She was a very bright student she even completed her 8th and 9th grade in one year. She topped in her school throughout.

She has a younger sister who is very much pampered by her mother and she was not very good at her studies. So when she used to top the school her mother used to be very angry and she used to beat her and force her to leave her studies.

She used to work for the whole day after coming from the school and then study at nights in the temple. After her 10th grade she got into the science stream and after her Higher secondary studies she got admission in a medical college to which her mother totally opposed but her elder brother supported her.

She completed all her education on scholarships and she went on to do her masters in gynecology and obstetrics. After completion of her studies she worked very hard and as she was all alone she had to face lot of difficulties. She then opened a big hospital and then it was inaugurated by her mother. But her mother was interested only in the money part and she used to give her whatever she wanted without any hesitation. she served her mother for 20 years and after so many years the mother one day felt very bad and told her that whatever she has done to her is unforgivable.

Then I was curious about her family her marriage we asked her what about her marriage?
She said that when she was in her first year MBBS her father expired due to Cancer and their family was not well off and she took the responsibility of settling down her brothers. She has seen to it that now all of them are well settled. She also said that her mother was never interested in looking out for a boy for her to get married .She felt bad about it.

She got married at the age of 50 years with a man who is already married and he fell in love with her after looking at the way in which she handled the work on her own. After her marriage she has adopted a child against the wishes of everyone in the family.

If she takes up some work she will look to it that it is completed and will not leave any work incomplete. She is very ambitious and takes her decisions on her own and has been quite successful in her field.

She suffered from an emotional shock when she lost her sister in law in a road accident.
She is also emotional by nature and cannot see anybody suffering and gives charity and money to the needy.

The other point was she has fear of darkness and of ghosts .As a child she mixed easily with people. She weeps when she is alone when she feels bad about something but this is not very frequent.

When she is angry she expresses herself immediately but afterwards feels guilty about why she has scolded someone.

The patient is free of pain. She is back to her work and feels good.
At this point the patient asked if I had given steroids. The most common question these days. The patient was explained the fact that we don’t use any such things and was also given a choice to test the medicine in a Laboratory.
The patient was put on Sac.lac (Placebo) and is under observation.

Arrival at the remedy

The selection of Lycopodium was based on the facts that the patient had a very difficult childhood.Inspite of the hardships she faced she didn’t give up and studied well. After the fathers death she took all the responsibility to see that all her family members are settled this is one of the important aspects of Lycopodium personality we find this in repertory under the rubric “Responsibility strong”.

This was reflected also in the fact that despite her mother hated her so much she didn’t hold any grudge against her and she looked after her very well till the time she died.

Also the fact that she was very ambitious and intelligent and she did what ever work she took in her hand and completed it.

The other symptoms that were considered were the marked craving for sweets and the flatulence that were so marked in the case.

A Case of Alopecia areata and Depression

The patient came to me with the complaints of total loss of hair in five different areas on the scalp. This had started in July 2000.

The hair fall was severe. Another interesting symptom was there is pain when the hair is touched ++.

The other complaints the patient described were Tension, Stress, Depression, Constipation, Severe headache, and Regular nose blockage, overweight and severe backache.

She had mentioned the following points; she is short tempered and gets angry easily, Sentimental, Emotional, Deep thinker, Helpful, Practical and intellectual. The patient wants to be very successful in life she is in to the fashion designing profession. The hurdles experienced were that things are not going in the way they should go.

She had lot of friends and she said she is close with one of them. She doesn’t open up easily to anybody. She is helpful to others but she doesn’t expect much because when she has expected she has got negative results. She says I am quite a social person.

On further case taking the patient told she becomes depressed, may be because she is at home. She gets hurt easily and is dissatisfied and unhappy .She weeps when she is alone and feels better after it.

At this point there was a short silence and then she said there was a break up with her close friend recently with whom she had plans to get married but she had never told these things to him. As he was unaware of her feeling he one fine day told her that he is carrying on with another girl and this was like a shock to the patient. The patient says she cannot forget him.

She becomes angry and retaliates, she feels like throwing things and breaking them but she controls her anger. If somebody tries to console her she just cannot bear it. It aggravated all her complaints.

The question of how the childhood was put? Then she told she wanted things her way but they never were the way she wanted. She wanted her parents to pamper her a lot but never did so .She felt very bad about it. She also had a younger brother who was pampered a lot. She feels there is nobody on her side.

She also has lot of suicidal impulses but she says she cannot commit it. The sleep is disturbed and she gets scary dreams like falling in a valley and somebody is pulling her down, Dreams of her X-lover. She has fear of animals, Dogs. She dislikes going in a crowd. There is lot of insecurity, which she was feeling. She dislikes wearing tight clothes.

What are Homeopathic Medicines made of?Homeopathic medicines are prepared largely from naturally occurring substances, plant, mineral or animal. Some substances are quite poisonous in their original, natural state. But when diluted as per homeopathic guidelines, they lose their toxicity and only the medicinal properties remain active. How are Homeopathic medicines tested?
Most countries where Homeopathy is well established have a central regulating authority. In the United States, for instance, there is the Homeopathic Pharmacopoeia Convention of the United States (HPCUS).

History of Homeopathy – A Holistic system of medicine

Homoeopathy was discovered in Germany 200 years back by Dr. Samuel Hahnemann.
He was a M.D physician. However, he was getting disenchanted with the kind of treatment doctors gave patients in his days. Most of it involved purging, one way or another. Patients were given laxatives, emetics or were made to bleed by applying leeches, in an attempt to cure diseases. Hahnemann gave up his practice of medicine and worked as a chemist.

He was also a scholar fluent in 14 languages. So he started translating the original medical work while he was translating one of the work of a doctor he came across the detail that the bark of the Cinchona tree could cure malaria. It was written it was biter that’s why it works on malaria.

Having a scientific mind, he decided to experiment. He ate a little of Cinchona bark and noted it produced symptoms similar to malaria. He gave the same bark to few of his friends and relatives and they produced malaria like symptoms. But he went one step further whenever a patient came with malaria; administration of this bark cured the disease condition!

He continued experimenting with different medicinal substances. He noticed that those that produced symptoms similar to a disease were capable of treating the disease. He formulated the law: SIMILIA SIMILIBUS CURENTUR. (LIKE Cures LIKE).Homoeopathy was born.

Why Homeopathy?
The above question comes to our mind when we are trying to switch off from one stream of medicine to another. We all know that every science has its positive aspects and its limitations.

Principles of Homeopathy

Some of the basic principles of classical homeopathy as described by its founder are are:
Diseases represent a disturbance in the body you require only a small stimulus to begin the healing process.
Seven eighths of all chronic diseases are due to suppressed skin or emotional condition. Even before it manifests in symptoms, the disorder has begun.
The totality of symptoms has to be considered. Detailed history taking is important to get the complete picture of the patient.

Treatment is individualized. No two persons have exactly the same constitution. Though symptoms they may produce may be similar, their reaction to the same medication may be different. For e.g. No two individuals are same not their sitting pattern, nor their behavior is same, they respond differently to the same stimulus. Even the monozygotic twins do not react in the same way. Even the veins of the leaves of the same plant are not the same. So the disease picture expressed by each individual is also different .So the medicine which will be required is the one that suits to the constitution of the patient.

Treatment is aimed not at the suppression of symptoms or treatment of the symptoms but it is the removal of the disease from within outwards and permanently.

The Four Fundamental Laws

The Law of Similars
This is the core concept of homeopathy. The cure mimics the symptoms of the disease. This works on the understanding that a “symptom” is not something that has to be controlled, but is an expression that the body is making an attempt to cope with the disease.

We get a fever when the body is already in the act of fighting a viral or bacterial infection. Homeopathic medicines therefore will make the body react as it would naturally when it tries to fight a disease.

For instance, if Malaria produces fever and chills, the homeopathic medicine for malaria will produce similar symptoms in a healthy person, and in an affected person, it helps to cure the disease –which means like cures like.

The Single Remedy
According to homeopathy one simple medicinal substance/mixture is given to the patient at a time. We do not require too many or to complicated medicines to treat the disease condition. The medication and the dosage will depend on each patient’s constitution.The understanding here is that the disturbance in the Vital level and this manifestation is in the form symptoms. These symptoms express inner disturbance which need correction.

The Minimum Dose
The smallest possible dose of homoeopathic medicine, just strong enough to create the stimulation of the system which is considered adequate. The medicines are diluted mainly with water or vehicle to one in 99 parts of vehicle and often this is further and further diluted multiple times to release the energy.

As homeopathic medicines are human specific and that is the reason why it is so effective in treating the sick individual who also expresses his symptoms individuality.

The Direction of Cure
Cure takes place from above downward, from within outward, from a more important organ to a less important organ; symptoms disappear in the reverse order of their appearance, the first to appear being the last to disappear.
For e.g. if you have walked a particular way in a room you will follow the same way to go out.

Suppose you have suffered from cold followed by asthma .The when cure takes place the asthma will get alright first then the cold will reoccur and finally the cold get better and the patient cured.

Another e.g. if patient suffered from skin problem which was suppressed and the patient develops insomnia when treated homeopathic medicines first the insomnia will get alright and then the skin eruptions will come back and finally everything will settle down that is cure as it is termed.
